TEAM COMMITMENT FORM

The Peninsula Predators welcome you to join our team for the 2012 season. By signing this form, you are making a commitment to play for the 2012Peninsula Predator team. You can not be placed onto the Predator team roster until we have a completed Team Commitment Form, among other forms that are required.

We want everyone to understand that our “product” is the sale of time; practice time, game time, etc. We have a very limited amount of this product to sell, and this is the reason we are asking you to please complete this form. Once you are selected to the Predator team through the selection process, and you commit to the join the Predator team by completing this form below, we have committed to you to sell you our product for the entire2012High School season. In addition, it is crucial to the other members of the Predator team that we secure a commitment to play from each rostered player.

WAIVER, RELEASE OF LIABILITY AND TEAM COMMITMENT

PLEASE READ CAREFULLY

I, the undersigned, parent or legal guardian, acknowledge the inherent risk involved in ice skating, and all sports relating thereto. Accordingly, in consideration of myself, or my child being allowed to participate in any skating activities and/or other activities at all Ice Center Enterprise rinks, I agree to the following:

1. I ACKNOWLEDGE AND FULLY UNDERSTAND THAT I (or child) WILL BE ENGAGING IN ACTIVITES THAT INVOLVE RISK OF SERIOUS INJURY WHICH MIGHT RESULT NOT ONLY FROM MY (or child) ACTIONS, BUT ALSO FROM THE ACTION, INACTION OR NEGLIEGENCE OF OTHERS; AND FURTHER, THAT THERE MAY BE RISKS NOT KNOWN TO ME OR NOT REASONABLY FORESEEABLE.

2. ON BEHALF OF MYSELF (or my child), I AGREE TO ASSUME ALL THE FOREGOING RISKS AND ACCEPT PERSONAL RESPONSIBILITY FOR MY OWN DAMAGES FOLLOWING SUCH INJURY.

3. ON BEHALF OF MYSELF (or my child), I RELEASE, DISCHARGE, WAIVE AND COVENENT NOT TO SUE ICE CENTER ENTERPRISES LLC, AND ALL THEIR RESPECTIVE AGENTS, AFFILIATES, ASSOCIATES, OFFICERS, DIRECTORS, OWNERS, AND EMPLOYEES (COLLECTIVELY “RELEASEES”) FROM DEMANDS, LOSSES OR DAMAGES ON ACCOUNT OF ANY INJURY, DEATH OR DAMAGE TO PROPERTY, CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY RELEASEES OR ANY OTHER PARTYS ACTIONS, INACTION, OR OTHERWISE; AND AGREE TO INDEMNIFY RELEASEES FROM ANY AND ALL THIRD PARTY CLAIMS CAUSED IN WHOLE OR IN PART BY MY (or child) ACTIONS.

4. ON BEHALF OF MYSELF (or my child), I AGREE TO ASSUME ALL FINANCIAL RESPONSIBILITY FOR COMPLETE PAYMENT TO ICE CENTER ENTERPRISES and/or PENINSULA PREDATORS FOR A FULL SEASON ($549.99) UPON SELECTION TO THE COUGAR TEAM AND SUBSEQUENT SIGNING. I UNDERSTAND THERE WILL BE NO PRORATION OF THE PENINSULA PREDATORS FEES UNDER ANY CIRCUMSTANCES, SUCH AS, BUT NOT LIMITED TO, INJURY OR ILLNESS. I FURTHER AGREE THAT BY SIGNING BELOW, I (or my child) IS AUTHORIZING THE PENINSULA PREDATORS TO INCLUDE ME (or my child) ON THE PENINSULA PREDATOR ROSTER FOR THE 2012 SEASON WITH ALL THE PRIVELEGES THAT BRINGS.

I have read the above Waiver, Release and Team Commitment, and understand that by signing below, I have given up substantial rights on behalf of myself or my child.
