Item / Action
1: Welcome & Introductions
  • Warm welcome and thank you to everyone attending this evening.

2: Recap:
  • Last time we met here at Pudsey Health Centre was 24th March 2016.
  • We held a Pudsey Locality PPG meeting on 14th June and cancelled our meeting in August.
  • When we met in March we gave you a staff update which we will do again today.
  • We discussed Friends and Family Test results and our patient survey.
  • We discussed 7 day access and we will give you an update later in the meeting.
  • We talked about changes to online access.
  • You suggested we introduce information via patient information screens which has been requested via The Primary Care Network who would fund this from The Challenge Fund.
  • Dr Hardaker discussed Physio First but the practice has decided not to proceed with this at this stage as we seen no benefit to the patient until the scheme is developed further.

3: Staff Update:
  • As you are aware Nurse Emma Ward left the practice to return to her family in Hull. We have now appointed an additional nurse Rachel Glynn who joins us on 21st November. Rachel currently works as a District Nurse in the Halifax area and has most of the skills required to carry out the role. She will undergo induction and further training to get used to our templates and systemsbefore seeing patients.
  • One of our nurses has now gone on sick leave for 6 to 12 weeks so until Rachel is starts properly we will be a little light on nurse appointments for a short while.
  • Our Assistant Practice Manager left at the beginning of September. Her hours were 8.30am to 2.30pm and due to business needs ie: being open from 7am to 7pm Monday to Friday we decided not to replace her. We chose to promote and develop two supervisors from existing staff (Tanya and Angela) one starts work at 8am and the other finishes at 6pm so we have greater cover and support in place.
  • Two GP Trainees joined us in February – Dr De Silva and Dr Racktoo both are in their final year. Dr Racktoo is now on maternity leave and will be back in practice in the summer.
  • Dr CM is GP Trainer and he explained that Dr Champanerir is also to be GP Trainer. She has completed her diploma and training is now being finalised.

4. CQC Inspection:
We had our CQC inspection on 12th July 2016 and our rating was ‘Good’ and is displayed in waiting areas and on our website.
The CQC asked for a couple of PPG members to come in on the day and thank you to those who attended to give feedback. I’m sorry we couldn’t invite you all but their time was limited.
CM thanked PS for the hard work which went in to organising the practice ready for the inspection.
There were no areas ‘requiring improvement’ but they commented that they could hear one of our nurses in the consultation room whilst they were speaking to patients in the corridor. The nurse does speak quite loudly so we have tried to improve this by removing the chair closest to her door. We also have the radio on to mask conversations coming from clinical rooms.
They commented that they liked the additional work we do:
BP Pilot:
The practice purchased 150 Blood Pressure monitoring devices to run a pilot to try to improve BP in patients. We’ve found that many readings are artificially high when completed in practice against lower readings when taken at home. Where possible patients should purchase their own machine but we are running the pilot to give to patients who are unable to buy their own. We have also created our own form for patients to record the BP checks at home which is being handed to patients and when the form is returned, this is reviewed by a doctor and entered onto the system.
Pudsey Locality work:
We are working closely with 4 other local practices (Hillfoot, The Gables, West Lodge and Sunfield) to improve thelives of patients with COPD, who are obese and who have or are a carer.
We are also working to create a weekend ‘hub’ so that our patients can access GP services 7 days per week in the local area.
Clinical Care Co-Ordinator role:
The practice has employed a Clinical Care Co-ordinator who contacts all patients on the 2% list (high risk) or if they are over 75 on hospital discharge in order to reduce re-admission by agreeing a care plan with the patient and sign posting themto local services.
5. Weekend Hub Working:
This forms part of our Pudsey Locality work with Hillfoot, The Gables, West Lodge and Sunfield practices.
The weekend hub will be based at The Gables and the latest information is that the weekend hub working will begin at the beginning of December. Looking at Saturday 2 x 4 hour sessions. Sunday 1 x 4 hour session.
PS will attend a meeting on Monday for an update but we understand that each practice will have an allocated number of appointments. If these appointments are not booked they will be opened up to the other four practices. There will be routine and book on the day appointments available.
There is an opportunity for all PPG members to attend the Pudsey Locality PPG meeting on 17th November at 2.30pm when the agenda will be:
  • Welcome and introductions
  • Background to funding and creation of the hub
  • Overview of service to be delivered
  • Questions and opportunity to view the hub

Patient who attended last locality meeting asked for a definite start time and if lunch will be available. She felt that the last locality meeting was poorly organised as The Gables patients were already there and lunch had been available but we weren’t informed. PS confirmed 2.30pm to start but would check and advise. / ALL 17.11.16

Developing Meaningful Patient Participation Groups:
Thursday 24th November 2016 at 10.30am.
PS will be attending the monthly Locality Development Session at Elland Road and as part of this meeting the Clinical Commissioning Group would like to invite one member of the PPG to attend from 10.30am.

8. Skype
CM confirmed that video conferencing would be available soon. The equipment is now in 4 of the GP consulting rooms and patients will notice 2 monitors in these rooms. We have not started doing skype consultations as yet.
9. Telephone appointments:
Patient asked if we were still doing telephone appointments? CM confirmed that any routine appointment can be changed to a telephone appointment.
10. Electronic Prescribing Service
PS confirmed that from 15.11.16 we will be moving to EPS which means patients will be able to nominate a pharmacy and the electronic prescription will be sent there rather than collecting a paper prescription from the practice. This should help to reduce foot fall at the practice and alleviate some of the work load for reception staff. Notices are in waiting areas and leaflets available in the practice and a note has been placed on the right hand side of prescriptions advising patients of this change. Pharmacies are also aware and should be asking patients where they wish their prescriptions to be sent to.
11. First Aid Training
Patient asked if GPs do any first aid training. CM confirmed we do not and this has to be purchased privately.
CM thought this would be a good opportunity to provide this training to and work with schools etc. although there had been a scheme in the summer where St Johns were providing this training.
12. Care Planning
Patient asked why she had received a ‘preparing for care planning’ sheet prior to her diabetic review. CM reminded that we are not able to discuss individual cases and went on to explain the ‘Year of Care’ which we are piloting for diabetic reviews at the moment.
Year of Care involves an information gathering meeting which is normally with the Health Care Assistant who takes bloods, weighs and completes foot checks etc. The results of these tests are sent to the patient with an example of what a ‘normal’ result should look like.
The results sheet also gives previous readings.
Making this information available to the patient helps to make them accountable and informed about their health and how they can try to improve it ie: stop smoking / loose weight. They are then invited to a meeting with the nurse to discuss their results and how they are going to work towards improving.
