Dance Guest Pass - 2017 Prom (May 6th, 2017)
The dance policy at Jefferson High School allows a JHS student to bring one guest to a dance, provided the following information is completed prior to the event. Guests are only allowed at dances and events with advance ticket sales.
The guest ticket may not be used without this form being submitted to the main office by 3:00pm on Wednesday, May 3, 2017 and be approved by the home school administration and the JHS administration. A guest must be at least enrolled in 9th grade. The guest must either be enrolled in high school or is a high school graduate. All Guests must be 21 years of age or under. Guests must present a picture ID at the door and have it available all night, upon request. Guests must submit a photocopy of their state/school issued ID when submitting this application. All guests not enrolled in high school must submit a copy of a government issued photo ID, indicating date of birth. Original IDs must be presented at the event to be admitted.

I realize that as a guest of Jefferson High School, I am required to abide by the rules and expectations for Jefferson High School students. I understand that failure to do so could result in my being removed from the dance.
