BAN JOHNSON PLAYER RELEASE FORM

I ______am hereby requesting my release from

______. I understand that the rules provide that each

(Team Name)

player can only request and receive an automatic release once during his

playing career in the Ban Johnson League. I also understand that under the

Ban Johnson rules that I am not free to sign with any team in the league but must

go through the process of being available to each team in the League in the order

established by the league rules and that I will only be eligible to play for the first

team that selects me. I also understand that I will not be eligible to play

for any other team in the League until all uniform items have been returned to the

team listed above.

This form must be signed by the releasing team manager, the player, the new team

manager once established and the Executive Director and must be attached to the

player contract turned into the League before I am eligible to play.
