Official Entry Form

> Judges will choose either a) one Hospital of the Year or b) one general-practice Hospital of the Year and one specialty-practice Hospital of the Year. Merit Award winners will be determined based on number of entries received.

> The hospital must have been completed between Nov. 20, 2012, and Nov. 20, 2016.

> All practices are encouraged to enter (small animal, equine, specialty), but they must be owned by a private practitioner(s) or privately owned corporation. Hospitals funded by foundations, government agencies, nonprofit organizations or publicly traded corporations are ineligible.

> Each entry must include a complete color photo “tour” of the hospital’s interior and exterior. Judges rely heavily on photos that show the features you consider unique and important and how the hospital enhances the efficiency of the team and the clients’ experience. See Section IV of the entry form for details.

1. Fill out this entry form (found at dvm360.com/hoy) on your computer.

2. Print and place in a three-ring notebook.

3. Place each page separately in a clear, nonfrosted sheet protector or photo-album page in this order: 1) entry form, 2) presentation drawing/site plan and 3) photos.

5. Use the following photo file naming convention for all photos. If there are multiple photos of the same room or area, please number them:

Please answer the questions below. Limit each answer to one page or less. You may need to write only a few sentences. Place each page in a nonfrosted sheet protector.

2.1 Design goals

What were your overall design objectives and specific design goals? How did this reflect your practice philosophy philosophy? Please explain any obstacles you overcame.

Section IV: Photographs

Include prints of interior and exterior photos with captions in nonfrosted plastic sleeves. Photos must be previously unpublished. Also, you must send all photo images on a disc or Flash drive in “.JPG” format. Photos must be at least 5 by 7 inches at 300dpi.

Please double-check the files to make sure they load properly on the disc or Flash drive before sending. Incorrect photo formatting may result in the disqualification of your entry.

Name your photo files on the disc or Flash drive using the convention below. If there are multiple photos of the same room or area, please number them:

NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.

ELIGIBILITY: VETERINARY ECONOMICS HOSPITAL DESIGN COMPETITION (“Contest”) is open to owners of veterinary hospitals that are privately owned and legally organized under the laws of the United States or veterinarians who are owners of hospitals as private practitioners of legal age of majority in their jurisdictions of residence (and at least 21) who are also legal residents of the 50 United States and the District of Columbia (excluding Puerto Rico and all other territories). Void where prohibited by law. Contest is subject to all applicable federal, state, and local laws. Failure to comply by providing incorrect information will invalidate your Contest entry, which will result in the loss of potential prizewinning entry.

Employees, officers and directors of Advanstar Communications Inc. (“Sponsor”) and its parent, affiliates, subsidiaries, advertising, promotional, fulfillment, and marketing agencies and their respective officers, directors, employees, and agents (collectively the “Released Parties”) and their immediate families (parent, child, sibling, and spouse) and persons living in the same households as such individuals (whether related or not) are not eligible to participate in the Contest.

By participating, participant agrees to these Official Rules and to the decisions of the judges (Advanstar Communications Inc.), whose decisions are final and binding in all respects. Void where prohibited by law, rule, or regulation. All federal, state, and local laws and regulations apply.

HOW TO ENTER: All entries must be made on the Official Entry Form and must meet all specifications described as required on such Official Entry Form. Download and complete the Official Veterinary Economics Hospital Design Competition Entry Form in Microsoft Word format at dvm360.com/hoy, and submit a notebook with required elements as outlined in the entry form. Mail your entry along with photos of the hospital you are entering in an envelope with proper postage affixed to: Veterinary Economics Hospital Design Competition, 8033 Flint St., Lenexa, KS 66214. All entries must be postmarked by November 26, 2016. Incomplete entries will not be accepted. Limit one (1) entry per hospital per year. All entries become the sole property of the Sponsor and will not be returned.

The entry must be the veterinary hospital owned by entrant, the entrant’s own original work, be in English, true and verifiable, be in keeping with the Sponsor’s image, and may not have been submitted in any other contest or won any type of award. The Sponsor reserves the right to disqualify any entry it deems to be offensive, inappropriate, not in keeping with Sponsor’s image, or otherwise not in compliance with these Official Rules, in its sole discretion. Released Parties (as defined below) are not responsible for any damage, theft, or loss to any entrant’s property resulting in whole or in part, directly or indirectly, from participation in the Contest.

Following the contest judging, potential winners will be notified by telephone, mail, and/or e-mail and are required to execute and return an Affidavit of Eligibility and Liability/Publicity Release (where permitted) within 14 days. Failure to comply within the time period specified, the return of any notification as undeliverable, or noncompliance with these Official Rules by a potential winner will result in disqualification and, at Sponsor’s discretion, a runner-up may be notified.

Winners will also be required to sign Sponsor’s Standard License Agreement granting Sponsor and its affiliated entities the non-exclusive worldwide perpetual license to edit, use, reuse, republish, reproduce, translate or otherwise distribute or disseminate the Material in any form or media (including digital electronic form and via the Internet), whether now known or hereafter devised (hereinafter, “Reuse” or “Reusing”); provided, and the worldwide perpetual right to exploit, edit, modify, and distribute such photographs as well as the entry (including photographs submitted as part of entry) and all elements of both in any and all media now known or not currently known (including in current and future advertising and promotions throughout the world in perpetuity without compensation, permission, or notification to winner or any third party.

JUDGING: All eligible entries will be judged by a panel of qualified judges in each of the following categories and criteria: originality of design (10 points), site plan (10 points), mechanical features (10 points), outpatient areas (10 points), plumbing features (10 points), inpatient areas (10 points), electrical features (10 points), quality of finish materials (10 points), and overall features (20 points). In the event of a tie in any part of the judging or in any category, an additional “tie-breaking” judge will determine the applicable winner based on the applicable criteria listed herein. Judges’ and Sponsor’s decisions are final and binding on all matters relating to this Contest. Sponsor reserves the right to not award any prize for any category if, in its sole discretion, it does not receive a sufficient number of eligible and qualified entries for such category. Winners will be announced on or about March 15, 2017.

PRIZES: Grand Prize: One potential winner’s hospital as described on the Official Entry Form will be named the Hospital of the Year Grand Prize Winner and will be featured in an article created by Sponsor that will appear in an issue of dvm360 magazine. Winner’s representative(s) authorized by winning hospital will receive complimentary registration to CVC Kansas City in August 2017 in Kansas City, Mo., held in conjunction with the Hospital Design Conference for two representatives from the practice. Grand Prize also includes roundtrip coach class airline transportation for the two hospital representatives between the major U.S. airport nearest winner’s residence and/or winning entry’s location and CVC Kansas City, and hotel accommodations for each representative (standard occupancy). Travel details are subject to provider terms and conditions.

Winners must make travel arrangements through Sponsor’s designated travel agent. All details of the trip not specifically listed herein will be determined by Sponsor. Winner’s are solely responsible for any expenses incurred in connection with the prize not expressly described above as being awarded including those expenses as may be incurred by winner’s representatives, and including without limitation, applicable federal, state or local taxes. Winner’s representatives must travel together on same itinerary, and each such representative must be at least 21 years of age and each must sign a liability/publicity release prior to travel. Approximate retail value for the Grand Prize: $2,000. Actual retail value depends on dates of travel and city of departure.

First Prize: An indeterminate number of Merit Award Winners, including one or two Hospital of the Year winners, will earn plaques of an Approximate Retail Value (“ARV”) of $100 each.

No substitution, transfer, or cash equivalent of prize is permitted except by Sponsor, which reserves the right to substitute a prize of comparable or greater value at its sole discretion. Any and all taxes on the prize, and any other costs and expenses associated with prize acceptance and use not specified herein as being awarded are the sole responsibility of the winning hospital. All prize details will be at the sole discretion of the Sponsor.

GENERAL: By participating, each entrant agrees: (a) to abide by these rules and decisions of Sponsor and judges, which shall be final in all respects relating to this Contest; (b) to release, discharge, and hold harmless Sponsor and its respective parents, affiliates, subsidiaries, and advertising and promotion agencies, and all of the officers, directors, shareholders, employees, agents, and representatives of the foregoing (collectively, “Released Parties”) from any and all injuries, liability, losses, and damages of any kind to persons, including death, or property resulting in whole or in part, directly or indirectly, from entrant’s participation in the Contest or any Contest-related activity (including any shipping, travel, or activity related thereto), the acceptance, possession, use, or misuse of the awarded prize or the use by Released Parties of any rights granted herein; and (c) to the use by Sponsor and its designees of his/her Veterinary Economics 2016 Hospital Design entry, name, company name, trademarks, logos, entry, biographical information, voice, performance, photograph, image, and/or likeness for trade, advertising, publicity, and promotional purposes in any and all media, now or hereafter known, worldwide and on the Internet and in perpetuity without compensation, notification, or permission unless prohibited by law and to execute specific consent to such use if asked to do so.

Released Parties are not responsible for late, lost, damaged, delayed, inaccurate, misdirected, incomplete, illegible, undeliverable, destroyed, mutilated, stolen, or postage-due entries or mail or for errors or problems of any kind, whether electronic, network, technical, typographical, printing, human, or otherwise relating to or in connection with this Contest, including, without limitation, errors that may occur in connection with the administration of the Contest, the processing or judging of entries, the announcement of the prizes, or in any Contest-related materials.
