1. / Q. What year does the NYSED-4.1 collect information on?A. The previous financial aid award year.
2. / Q. What do I do if I am unable to submit the NYSED-4.1 by the due date?
A. Call the Office of Research and Information Systems (ORIS) at (518) 474-5091, email
the office at , or fax the office at (518) 474-1907 and provide an
explanation as to the reason why the due date cannot be met and provide an estimated data
submission date.
3. / Q. Why do I need to submit a NYSED-4.1 to ORIS?
A. ORIS uses the financial aid data submitted by the college to calculate the average
net price and to fill numerous internal and external data requests.
4. / Q. What happens if I do not submit the NYSED-4.1 to ORIS by the due date?
A. The College will be dunned for the missing form and the status in HEDSLive will not be
changed to accepted and the previous year’s data will be entered as a HEDS Estimate.
5. / Q. Can I submit a NYSED-4.1 via HEDSLive?
A. Currently the NYSED-4.1 must be submitted using Access or via a Word Document.
ORIS anticipates making the NYSED-4.1 available as a HEDSLive form in the future.

Large changes between fields from year to year should be verified.Financial Aid categorical aid was once collected from HESC.
Subtotals are unduplicated counts of recipients.
Colleges should be called when they report a lot of recipients receiving other aid.
No PELL data is available from 2005-2009.
Data covers the amount recipients received during the prior year.
Form is due September 15th.
Dunning begins October 1st.
