2013 Young Achiever Award – Chinese Institute of Engineers/USA Dallas-Fort Worth Chapter
Application Form
This award is designed to recognize young students with an excellent academic record, strong interest in engineering, active community services and extra-curricular involvement. A qualified applicant must:
Be a 10th or 11th grade student in a DFW area school during the Spring 2013 semester.
Be in the top 10% of current school academic score ranking
Be a dependent child of a Life Member of CIE/USA-DFW Chapter. (For membership, see:
Have not won Young Achiever Award before.
Present at the CIE Annual Convention banquet for the YAA award ceremony to receive the award on Saturday, August 10, 2013 at Hyatt Regency North Dallas Hotel (701 E Campbell Rd, Richardson, TX 75081).
To apply, please follow the instructions below:
- Email the following documents in PDF format no later than June 17th, 2013:
- Complete this application form with required information as detailed as possible.
- Write a one-page biography that best describes your academic achievement, strong interest in engineering, active community services, extracurricular activities and personal aspiration.
- Academic Records:
- Official High School Transcript
(1) An official transcript includes the school’s official sealed or a school officer signed your class rank, class size and high school coursework.
(2) The transcript should show coursework and rank through the end of Spring 2013 semester. If the latest transcript can not be obtained, please add a statement from your school counselor.
(3) If your high school does not rank class members, please add a ranking policy statement from your school, a copy of your school’s profile and a GPA or grade distribution.
- For completed College Coursework – including dual credit coursework – while in high school: Submit official transcripts of the credit issued by the college or university.
- Community Services Materials:
- Accumulate community service hours and proof materials
- Write a one-page essay that best describes your leadership potential and the impact of your community service.
- Recent portrait photo in JPEG (headshot with formal dress and clear background; resolution higher than 720x480).
- Mail the following originals and must be postmarked no later than June 17th, 2013, to

- Request TWO recommendation letters: ONE from your teacher; Another ONE from your community services leader using the enclosed templates.

- Recipients will be announced on/or before July 15th, 2013.
The award will be presented at the Dinner Banquet of CIE/USA DFW annual convention on Saturday, August 10, 2013 at Hyatt Regency North Dallas Hotel (701 E Campbell Rd, Richardson, TX 75081). Names and photographs of all recipients will be published in the convention proceedings and will be posted at the annual convention. The recipients will also receive a Life Membership from CIE/USA-DFW.
