Salary £18,564.00 per annum (£10.20 per hour)
Hours per Week 35 hours per week
Temporary subject to funder approval and then fixed term contract until 30 June 2019
Annual Leave 25 days plus 10 Statutory holidays (pro-rata for part time / term time roles)
Reports to Community Services Development Manager – localities and commissioning

Henshaws:
Henshaws Community Services enable people of all ages living with sight loss and a range of other disabilities to make informed choices about their future.
As a Community Enablement Officer you will support people throughout their journey offering expert support, guidance and skills, helping people to fulfil their potential.
Independence and friendship are at the heart of what we do to help people find the confidence to go beyond their expectations.
Henshaws community service has received funding from the Big Lottery Fund to deliver Henshaws Pathway to Wellbeing project in Manchester. Key duties will be to develop, deliver and co-ordinate Henshaws wellbeing programmes which support Henshaws Pathway to Independence delivery model.
Key responsibilities:
· To deliver and coordinate Henshaws wellbeing programmes with specific focus on; person centered planning, independence matters and friendship matters programmes.
· To deliver our wellbeing programmes in a community based setting; one to one Information, Advice and Guidance sessions, Henshaws Living with Sight Loss programme, Henshaws gentle exercise classes and co-ordinate and support a volunteer led Friendship Matters programme.
· To liaise with, promote, support and refer into Henshaws counselling service and signpost to other relevant services
· To provide people living with sight loss with a bespoke assessment and personal plan that supports people to live independently in their community.
· To promote Henshaws pathway to wellbeing programme which forms part of Henshaws pathway to independence delivery model to partners and other community groups to increase referrals into the project.
· To provide information, support and training on the specialist aids and equipment available for Henshaws clients and to ensure they can use the equipment appropriately.
· To organize and recruit local volunteers to support the delivery of Henshaws Friendship Matters groups and to increase awareness locally of Henshaws.
· To data input into Henshaws systems to ensure all beneficiary information is accurately captured and maintained.
· To provide monthly monitoring and reporting against key performance indicators to meet the programme’s specific outcomes and objectives to comply with our funding agreements.
· To work closely with the partner venues and the local authority sensory team to ensure all project outcomes are met effectively
· To network with other relevant Health and Social Care agencies to provide an holistic support package to people living with sight loss who could have other health conditions.
· To liaise with other internal and external staff in the development and delivery of Henshaws Community services.
· To promote Henshaws Community Services (Vision Support Services) in conjunction with the Vision Support and Development Manager.
· Reinforce agreed rehabilitation actions and good practice with centre staff and others who support service users in their rehabilitation needs.
