1.  Introduction

1.1  This service is available to all patients registered with NHS North Lincolnshire (NHSNL) general practices as identified in Appendix 2. Patients are at liberty to refuse this service. Patients who pay for their prescriptions should be referred to a pharmacy to purchase medicines in the usual way. For these patients medicines will be charged at the price of a prescription or the purchase price if less than the prescription charge. The pharmacy will provide advice and appropriate medicines when clinically necessary.

1.2  The service is only available for the minor illnesses identified in Appendix 6.

1.3  Only community pharmacies who are committed to making staff available to provide the service and who have completed the necessary training will be included in the minor ailment service.

1.3.1 a) Participating Pharmacies should have a regular pharmacist(s)

b) Participating Pharmacies should have an approved private consultation area for this service.

c) Participating Pharmacies should have a named member of staff to manage the overall service.

Participating pharmacies are listed at Appendix 1.

1.4  The contract for pharmacies to participate in the scheme will commence on 1October 2009.

2.  Transfer of Care

2.1  Patients presenting with a minor illness at the GP surgery can be offered this service. Patients may also self refer.

2.2  Patients presenting at a participating community pharmacy confirmed as being registered with a NHSNL surgery will receive the level of care as laid out in this service specification.

2.3  Patients from NHSNL surgeries presenting at a participating pharmacy with a passbook will receive the level of care as laid out in this service specification.

2.4  All patients must present to the pharmacy in person. For patients under the age of 16 the parent/guardian can accept transfer into the scheme on behalf of the patient. The parent/guardian must always bring the child with them to the pharmacy in order for an assessment to be carried out by a pharmacist/pharmacy staff member.

2.5  If the patient does not present in person, the patient will not be eligible for the scheme and they will be advised to access medical care through the normal procedures.

2.6  A passbook must be presented in order to receive services under this agreement.

Duties of Participating Surgeries (Appendix 2)

3.1  All patients requesting appointments for symptoms matching the criteria identified can be offered this service.

3.2  Patients presenting in person accepting the service should be issued a service information leaflet and a list of participating Pharmacies.

3.3  Monitoring of inappropriate use or abuse of the service must be undertaken and if identified should be notified to the NHSNL Medicines Management team.

3.4  Surgeries should co-operate and liaise with Community Pharmacists to activate the rapid referral procedure when required (see section 4.9).

3.5  Participating NHSNL surgeries should display official posters and provide leaflets promoting the service.

3.6  Patients accepting transfer by telephone will be advised to take evidence of identity or Passbook if already registered on the scheme to one of the participating pharmacies. In the absence of an NHS card or passbook the pharmacy may telephone the surgery to confirm a patient is on the practice list.

3.7  For patients under the age of 16 the parent/guardian can accept transfer into the scheme on behalf of the patient.

Duties of Participating Community Pharmacies (Appendix 1)

Patients should only be accepted into the service if the pharmacy can confirm their identity and has reasonable proof of registration with a NHSNL surgery. The pharmacy will provide the patient with the passbook and the same pharmacy will reissue a new passbook as required. The patient can access any pharmacy in the scheme.

4.1  Children under 16 years old, who self refer, without parental agreement, should be Gillick competent to receive this service.

4.2  All participating pharmacies will provide a professional consultation service for patients registered with a NHSNL surgery who present with one of the specified conditions. (Appendix 6)

4.3  The Pharmacist will assess the patient’s condition. The consultation will consist of:

4.3.1  Patient assessment.

4.3.2  Provision of advice.

4.3.3  Provision of medication, only if necessary, from the agreed formulary appropriate to the patient’s condition.

4.4  Normal rules of patient confidentiality apply.

4.5  The Pharmacy should ensure that the patient has completed and signed the declaration of exemption of Prescription charges and confirm this in the usual manner. (Appendix 3).

4.6  A record of any supply should be made on the Pharmacy Patient Medicine Record system.

4.7  All participating pharmacies will provide agreed medication in accordance with the Medicines Act 1968 and within the medications product licence.

Prescription Only Medicines must only be supplied under appropriate patient group directions (PGD’s).

4.8  If a patient presents more than twice within any month with the same symptoms they should be referred to their GP, dentist or optometrist and unless the symptoms meet the criteria for a rapid referral the patient should be advised to make an appointment in the normal manner. The Pharmacy Referral Form should be completed (Appendix 4).

Pharmacists should report significant interventions to the patients’ GP, dentist or optometrist (Appendix 4)

4.9  Rapid Referral Procedure – If the patient presents with symptoms indicating the need for an immediate consultation with the GP, dentist or optometrist the pharmacist should ring the appropriate professional and make an appointment for the patient within the appropriate time frame.

If the surgery is closed and/or the symptoms are sufficiently severe the patient should be advised to contact the on-call doctor at the GP Out of Hours Service, Emergency Dental Service or optometrist (if available) or attend A&E (if appropriate) immediately.

4.10  If the pharmacist suspects that the patient and/or parent are abusing the scheme they should alert NHSNL Medicines Management Team or the patient’s GP.

4.11  Pharmacies will be required to complete a monthly summary sheet and submit it to the PCT at NHSNL, Health Place, Wrawby Road, Brigg, DN208GS. This will allow for evaluation of the service, reimbursement of drug costs and feedback to pharmacies and practices. (Appendix 5).

4.12  Copies of the Data Collection (Appendix 5), Referral/Reporting Forms and completed Pass Books should be retained for audit purposes by the pharmacy for at least one year.

4.13  The pharmacist must ensure that adequate levels of stock are maintained.

4.14  All medication supplied should be in original packs and must contain a patient information leaflet. There is no need to label medication.

4.15 The pharmacist must inform the PCT if for any reason they are unable to deliver the service.

5.  Service Funding and Payment Mechanism

The pharmacy will be paid according to the following schedule:-

5.1  Agreed cost price of medicine + VAT.

5.2  £3.00 professional fee per consultation.

The summary recording forms (Appendix 5) should be returned to the PCT office by the seventh of each month to enable payment at the end of the month. Pharmacists are advised to retain a copy of the summary forms.

6.  Monitoring and Evaluation

Pharmacies and surgeries will be expected to participate in monitoring and evaluation to show:-

6.1 Any changes to GP prescribing data

6.2 Cost and volume of Pharmacy prescribing and indications treated

6.3 Impact on GP appointments and time

6.4 Attitudinal survey of:-

a)  GPs

b)  Receptionists

c)  Pharmacists

d)  Patients
