Community Health Worker Extraordinary Service Award
Purpose
The purpose of the Community Health Worker Extraordinary Service Award is to recognize and acknowledge outstanding service, dedication and excellence within the profession. All community health workers (also known as village health workers, public health technicians, promotoras, community health advisors, outreach workers, health auxiliaries, health workers, lay health workers, health agents, health promoters, family welfare educators, health volunteers, school health aides, diabetes community health workers and community health aides) are eligible for nomination.
Community Health Workers Definition
The CHW Special Primary Group Interest Group (SPIG) has adopted the following definition of the CHW:
A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison, link or intermediary between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

Nomination Process and Requirements
§ The nominee must be a community health worker
§ Must have a minimum of two years experience as a Community Health Worker
§ Submit the nomination form
o Include the full name of the nominee on the nomination form. The description of the nominee’s contributions or achievements must be filled out, as well as how the nominee has influenced and contributed to their specific community service area(s).
§ One single spaced, typed page. Margins should be set at 1 inch, and font should be 12 point and include:
o Explanation of work/job functions and duties
o Length of service as a community health worker
o Description of the population the nominee serves
o Description of community involvement within the Community Health Worker role
o Identify cultural sensitivity, diversity, and inclusion efforts
o Description of problem solving skills
o Two letters of recommendation – one letter must be written by the nominee’s supervisor (or project/program director if the nominee is an unpaid Community Health Worker)

Selection of Award Recipients
The award winner will be selected by the award committee. The winner will be announced on August 21st, 2013, during the Sixth Annual Community Health Worker Conference at Grand Rapids Community College Wisner-Bottrall Applied Technology Center. The award winner will receive a commemorative plaque, and his/her conference registration fee will be waived.
