Please ensure that you have included the following with your application as required:

a / Originals of any certificates or diplomas referred to in Sections4-7.
b / A recent performance assessment or reference from your current or most recent employer – not older than two years before the date of application. (If you do not have a recent performance assessment or reference, a reference is required from you current or most recent employer or an alterative referee – see Section 8 of this application form.)
c / Any additional separate sheets as may be required.

e / A copy of your passport information pages (personal identification details and expiry date).
f / Two recent passport type/sized photographs. Your photographs must meet the following requirements.
  • be identical
  • be of you on your own
  • be taken within the last three months
  • be taken against a plain cream or light coloured background and free from shadows
  • be in clear, sharp focus, with a clear difference between your face and the background
  • be of you facing forward, looking straight at the camera showing your full head
  • be a close-up of your head and shoulders, showing you with a neutral expression with your mouth closed
  • be taken with nothing covering your face that may obscure the outline of your eyes, nose or mouth
  • be taken with your eyes open, free of red-eye and clearly visible (without tinted lenses or hair across the eyes)
  • be taken showing you without any head covering unless you wear it for religious beliefs or medical reasons
  • be free from reflection or glare on your glasses (where possible, we recommend that you remove your glasses)
  • be in colour, not black and white
  • be printed on plain white photographic paper, untorn, uncreased and unmarked
  • be of professional standard photographic and print quality
  • be 45 mm high x 35 mm wide
  • be clear of writing on the front and back
