2.I have known the applicant (name) for (state period) as a (state relationship -personalfriend/colleague/neighbour etc)
3.I first met the applicant in (year) when (state the circumstances in which applicantand deponent met and nature of relationship since).
- There is not and has never been any business or professional relationship between the applicant and myself. OR Apart from the relationship disclosed in paragraph (number) there is not and has not been a business or professional relationship between myself and the applicant.
- I am not related to the applicant by blood, affinity or adoption.
6.(If the applicant has disclosed an offence, or any matter relevant to fitness for admission, such as a Centrelink over payment or academic misconduct, in his/her affidavit, the deponent should include a paragraph stating that they have read the applicant’s affidavitand have taken the matter(s) disclosed into account whenassessing the applicant’s fame and character).
7.I consider the applicant to be a person of honesty and integrity and I believe that this opinion is shared by those with whom (he/she) comes into contact. In my opinion, the applicant is a person of good fame and character.
