The Patient Protection and Affordable Care Act (PPACA) will expand access to healthcare to an estimated 32 million Americans. The ability of our healthcare system to support this influx will depend on payment and delivery system reforms to foster increased coordination and quality of care. Primary Care and the Patient Centered Medical Home (PCMH) will be an integral part of these reforms. PPACA provisions include incentives and resources to assist expansion of new and existing PCMH pilot programs. In the development of new programs, it will be imperative to examine what has worked well, and what needs improvement.
The Capital Region is fortunate to have model examples of ongoing and innovative PCMH pilot programs. CDPHP recently launched an expansion of its original program that began in May 2008, to encompass a total of 24 area practices. During this interactive session, we will be focusing on the state of the evidence, CDPHP’s experience in investing and implementing a program, and a local Primary Care Provider’s perspective on the physician practice experience.
