Project Description: (maximum 100 words)Provide a summary of the innovation, describing the initiative’s primary intent and outcome, as well as key stakeholder groups that it benefitted. The first sentence of the project description should serve as a summary statement and be able to convey key priorities if it were to stand alone. As appropriate, this statement should address the administration, funding, and background (context) of the project.

Objectives: (100 words)

  • Use statements to indicate the most significant objectives of the innovation.
  • Choose no more than four objectives to indicate the major components of the innovation.
  • Indicate how each objective will be evaluated (how you will know that you have achieved the objective)
  • Use the next section three sections to expand on the evaluation/assessment of the innovation.

Quantifiable/Qualifiable Outcomes: (100 words)

  • Use statements to indicate the outcomes that have been realized.
  • Use both quantifiable (numbers) and qualifiable (opinion or survey- based) outcomes.
  • As appropriate, indicate future intended outcomes.
  • Include no more than four outcomes.

Challenges/Problems Encountered: (100 words) Describe challenges (institutional or external) that had to be overcome in order to implement the innovation. As appropriate, discuss adjustments that had to be made in addressing the problems encountered.

Evaluation Approach:(75 words)Describe how the evaluation process was approached from an administrative standpoint. How were the major outcomes and the data elements needed to indicate success arrived at? Who is responsible for assessment related to this specific project?

Potential for Replication: (75 words) Every innovation has the potential for replication, but adjustments or changes are often needed for the program to be successful elsewhere. What adjustments should be considered in applying the innovation in another setting? Were there special circumstances pertaining to the primary stakeholder groups impacted and/or other factors in the environment that warrant consideration?
