Volunteer Activity Pre-Approval Form

Any volunteering activity not listed on the BHS Student Volunteer Opportunities list must be pre-approved to ensure that it will qualify for school credit. Completed forms must be returned to either the Guidance Resource Center, or to the Student Volunteer drop box near the GRC door. You will be notified regarding the status of your volunteer activity via email. Any forms submitted during the summer break will not be considered until after school resumes in late August.
