Individual Placement Description - Basildon and Thurrock University Hospitals NHS Foundation Trust
Placement / F1 General SurgeryThe department / BTUH NHS Foundation Trust is a University Hospital affiliated to UCL, situated 30 miles to the east of London on the North Bank of the Thames Estuary. The Trust has an established reputation amongst the surgical community for the delivery of supervised training and is popular amongst the trainees. Basildon is the largest teaching hospital outside London for UCL students, consequently there are many opportunities to participate and deliver teaching to medical students.
The General Surgical department provides General Surgical on call services for the population of Basildon and Thurrock and Elective surgical services in Upper GI, Colorectal, Vascular, Breast, Endocrine, Urology and Paediatrics.
The General Surgical department is structured into three ‘superfirms’, each has two or more Consultants supported by at least one Specialty Doctor, two Specialist Registrars, 2 F2/Core trainees and F1s. There are a number of specialty nurses who also support the Multidisciplinary services. The firms are Colorectal, Vascular and Upper GI with Breast. There is a separate Urology Firm with 3 Consultants, 2 Specialty Doctors, one SpR, one F2 and 4 F1s.
The type of work to expect and learning opportunities / F1s are attached to specific surgical firms and expected to take an active role in the day to day management of the firm. They are responsible for documenting progress in the Clinical Record, organising investigations requested by the team and collating and communicating the results of those investigations. FY1s are expected to undertake simple ward based procedures with supervision as necessary, but are also encouraged to attend the operating theatre. Simulation centre training is available for a wide range of surgical skills from simple iv cannulation to laparoscopic and endoscopic simulation. Once competent in simulation these skills can be transferred to clinical practice.
There are regular educational meetings within the department, in addition to the F1 programme lectures held on a Monday. This includes a monthly M&M and Surgical audit meeting, and weekly SpR led MRCS teaching on a Thursday morning.
National courses are also held in the simulation centre, these include Basic Surgical Skills, Advanced Trauma Life Support, the Basildon Trauma Course and Core Laparoscopic skills .

Main duties of the placement / When on call the F1 is responsible for the management of the surgical inpatients. F1s are also encouraged to participate in the assessment and management of emergency admissions and are expected to attend all trauma calls, where acting as the scribe they observe and record the activities of the trauma team. All documentation is subsequently reviewed by the trauma team leader which allows an excellent learning opportunity. Each F1 is allocated a clinical and educational supervisor. Within the department F1s are encouraged to develop independence at a level commensurate with their competence and confidence. As the year progresses F1s are allowed increasing levels of responsibility whilst still being offered close Consultant lead support and teaching.

We provide an extensive range of acute medical services at Basildon and Orsett Hospitals, plus x-ray and blood testing facilities at the St Andrew's Centre in Billericay. We also provide dermatology services across the whole of south Essex.
The Essex Cardiothoracic Centre is also part of the Trust, providing a full range of tertiary cardiothoracic services for the whole county and further afield.

Placement / F1 General Surgery w/ BreastThe department / Mr Salih/Mr Chicken are Breast and General Surgeons, Mr Taylor who also undertakes colorectal surgery works in the breast unit with the addition of a Locum Specialist. The firm currently has one SpR equivalent, one F2 and one F1.

Main duties of the placement / The breast unit sees 2 week wait cancer and symptomatic breast patients every week. They are seen within a multidisciplinary one stop clinic, with radiology, histopathology and oncology support. The unit performs primary reconstructions with the support of the plastic surgeons from Broomfield.
Mr Salih and Mr Chicken participate in the General Surgical on call rota so the trainees benefit from the exposure to the normal range of emergency admissions.

Placement / F1 General Surgery w/ ColorectalThe department / There are 5 Colorectal surgeons, Mr Taylor, Mr Ashraf, Miss Lovett, Mr Zammit and Locum Consultant. They are supported by 2 Associate specialists, 3 SpRs, 1 core trainee, 3 F2s and 5 F1s.
The type of work to expect and learning opportunities / The unit performs over 200 colorectal procedures for Cancer per annum. The recent appointment of two new laparoscopic surgeons will increase the percentage of cases performed laparoscopically.
Basildon is a Bowel Cancer Screening Centre and there is a close working relationship with the gastroenterology firms. The weekly Colorectal cancer MDT is attended by surgeons, physicians, oncology, pathology and radiology and supported by the Colorectal nurse practitioners. There is also a fortnightly Inflammatory Bowel Disease MDT where complex patients are reviewed with radiology and histopathology support and management planned.
SpRs attend endoscopy lists, undertake regular operating and Consultant led clinics.

Main duties of the placement / The F1s are encouraged to work independently on the wards following the guidance of senior staff. They are encouraged to attend theatre whenever possible. They are allocated to pre-assessment clinics which are proforma driven, supported by dedicated nurse practitioners and supervised by a Consultant anaesthetist.
