MAIN PURPOSE OF JOB:
To develop and manage a regional programme of change to support integrated commissioning of care and support in West Wales and the implementation of preventative services, including Information, Advice and Assistance.
PRINCIPAL ACCOUNTABILITIES:
1.  Work with a range of partners and stakeholders across the region to deliver strategic priorities agreed by the Regional Partnership Board in relation to integrated commissioning and prevention.
2.  Establish integrated approaches to commissioning of care and support services, including:
·  The development of shared commissioning strategies for key services which will inform the Area Plan in response to the regional Population Assessment
·  A shared, strategic approach to market analysis and facilitation
·  Development of the market, including growing social enterprise, cooperatives and user-led services
·  Development of common contracts with care providers
·  Consistent approaches to contract monitoring and quality assurance
·  Upskilling of commissioning capacity in the region and identification of opportunities for integrated working
3.  Support the implementation of effective, high quality prevention services within the region by:
·  Working with appropriate partners and stakeholders to develop a shared prevention framework with clear strategic objectives and supported by consistent standards and outcome measures
·  Supporting the evaluation of different approaches across the region
·  Identifying and sharing innovation and best practice from within and outside the region
·  Rolling out Dewis and Infoengine as the principal information portals for West Wales, promoting them widely, ensuring they are effectively maintained and supporting the development of links with other systems such as the NHS 111 service and Family Information Services
·  Working with NHS colleagues to ensure that the Cluster Development Programme within primary care supports agreed strategic objectives and that funded programmes contribute effectively to a whole systems approach to prevention
4.  Develop detailed programme plans for each of the above
5.  Establish robust programme governance and identify, deploy and manage programme resources, including an allocated regional budget.
6.  Develop and maintain a comprehensive risk register and identify and implement appropriate mitigation action
7.  Commission external capacity/ support to assist delivery of the programme as required
8.  Liaise with senior Welsh Government officials on a regular basis to report on progress, obtain policy advice and share emerging practice
9.  Link with developments in other parts of Wales and import appropriate learning
10.  Report regularly on progress to partner organisations and the Regional Partnership Board, including activity and financial updates
11.  In partnership with the Regional Collaboration Unit, identify links with other regional programmes such as those supporting the development of pooled funds, to ensure synergy and avoid duplication
12.  Develop and deliver a communications plan to support the programme, linked to the regional communications strategy and ensuring key stakeholders are appraised of its aims, progress and outcomes
13.  Provide a regional point of contact for the programme and advise colleagues as appropriate
14.  Promote the benefits of regional working and the wider West Wales regional programme
MAGNITUDE OF JOB:
This post will be a high level appointment requiring an individual with evidenced abilities in project management and the ability to deliver key priorities identified by the West Wales Care Partnership. The post holder will be required to broker consensus and obtain buy-in from a disparate range of stakeholders to a key programme of change and maintain project momentum. There will also be a requirement for frequent liaison with senior colleagues in Welsh Government and other agencies to obtain specialist advice and feed back on progress.
The post holder will need to adjust and reprioritise workload to respond to changed local and regional needs, translate nationally-driven imperatives into regional and local initiatives, produce complex and specific work within tight timescales and be able to convey the need for meeting crucial deadlines across partner organisations. This will be within the context of frequently changing and often conflicting requirements and deadlines. The post holder will be to manage this complexity under their own initiative and with minimal supervision.
