*Improvement trend in one of the following data elements:
  • Self-Assessment Survey
  • Staff retention data
  • Climate Surveys
  • Special Education Referral Information
  • Attendance
*Improvementshould be reportedusing 3 consecutive years of current data. Additional data showing a pattern of improvement over a period of several years in specific data elementsmay also be included. If there has been a decline in growth over past three years, include past data to indicate trend. Provide a written explanation of the decline.
