CHECKLIST FOR SCHENGEN VISA–SEAFARERS

-Fully completed in English or Greek and signed by the applicant.
2. / Passport
-Valid for at least 3 months after the intended date of departure from the Schengen area, issued within the previous 10 years and containing at least 2 consecutive blank pages.
-Residence permit (where applicable), must be valid for at least 3 months after the intended date of departure from the Schengen area.
-Photocopies of data page, residence permit and previous Schengen and UK visas are required (in case of a new passport, photocopies of the old passport).
3. / One (01) recent color photograph
-Not older than 6 months, size 3.5 cm × 4 cm, white background, no head or face cover(s) that prevent(s) full visibility of the facial features.
4. / Valid Seaman’s Book
5. / Medical fitness certificate
-Certifying that the applicant is free of any disease capable of putting public health at risk, according to the international situation and the World Health Organization, as well as of other contagious, infectious or parasitic diseases which would require the adoption of public health protection measures (see attached template).
6. / Travel medical insurance
-Covering the entire period of intended stay, valid for all Schengen States, covering any expenses which might arise in connection with repatriation for medical reasons, urgency medical attention, emergency hospital treatment or death during your stay (min. coverage €30000).
7. / A copy of the seafarer’s contractor letter of appointment.
8. / Ticket
-Copy of flight ticket or flight reservation.

9. / Application by the shipping company (or a duly authorized shipping agent,
required to have a shipping agent permission by the competent port authority)
-Indicating the seafarer’s full details including:
- The seafarer’s identity (Name, Surname, Nationality, Date of birth, Passport number, Rank/Grade)
- Date of applicant’s entry into the Schengen/Greek territory and signing-on
- Details of the shipping agent of the country where the visa is to be issued
- Vessel’s name, registration number, flag under which the vessel is sailing, and IMO number (where applicable)
- Scheduled date of vessel’s arrival in the port where the seafarer is to sign on
- Scheduled date of vessel’s departure
10. / Formal declaration signed by the shipping company or its representative or a duly authorized shipping agent
-In the form of a solemn declaration as per Law 1599/1986 (legalization by public authority not required), confirming that the seafarer signs on upon an invitation, as well as their responsibility for the expenses of the stay and, if necessary, for the repatriation costs of the seaman.
Please note:
-All documentation has to be submitted in English or Greek language.
-Applicants will be required to submit their biometrics (fingerprints, digital photo) at the time of application.
-The Embassy reserves the right to ask for additional documents.
-Depending on the applicant’s nationality, visa processing time at the Embassy may vary from 2 to 15 days.
-Submitting an application without the complete set of documentation according to the above checklist may result to the rejection of the visa application.
