POSITION TITLE PROJECT SEARCH LIAISON

DEPARTMENT EMPLOYMENT

POSITION TYPE NON EXEMPT

REPORTS TO Director of Consumer Services

POSITION PURPOSE Insure strategic planning with school system, communities, providers and families to improve transitional outcomes for disabled students.

ESSENTIAL FUNCTIONS

1. Provide ongoing classroom services, work with and assist staff in observing social, work and community skills taught and practiced with eligible students.

2. Adhere to policy and procedures mandated by Project Search

3. Participate in the development of a summer transition camp.

4. Provide information needed for student/Parent/Caregiver to assist in the selection of services after high school.

5. May requires a personal valid driver’s license and current vehicle insurance in employee’s name to implement job duties. Driving may also be necessary to transport clients as needed.

6. Follow all agency policies and procedures regarding the use and release of confidential information and confidential PHI (protected health information)

7. Adhere to Universal Precautions for all functions, uses protective equipment as necessary and required.

8. Maintains a positive and enthusiastic attitude.

9. Develop community and business involvement.

ADDITIONAL RESPONSIBILITIES

1. Coordinate with Director of Community Employment coverage for Project Search schedule.

2. Provide 1 on 1 or small group training to address individual client needs such as those indicated on the individual program plan.

3. Shall perform any other job related functions as assigned.

KNOWLEDGE AND SKILLS

Must have the ability to prioritize tasks and to organize time and duties. Good interpersonal skills. Computer literate.

Project Search Liaison

PHYSICAL REQUIREMENTS

1.  Speaking, hearing and visual ability to communicate.

2.  Bending, stooping, reaching, lifting and carrying to demonstrate job tasks at a variety of job sites.

3.  Sitting and standing are required.

MENTAL REQUIREMENTS

1.  Ability to observe, synthesize and record pertinent data

2.  Write reports and work independently.

EDUCATIONAL REQUIREMENT and JOB EXPERIENCE

High School Diploma or GED equivalent. One to two years’ experience in training job skills to others or experience with individuals with disabilities.

ADDITIONAL INFORMATION

1.  Flexible schedule to meet needs of consumers and educators. Position to follow LaPorte Community school year schedule.

2.  Must adhere to safety requirements.

3.  Must provide Michiana Resources, Inc. with Indiana valid driver’s license and up-to-date proof of vehicle of insurance for the vehicle being driven for Michiana business.
