The post holder will be an autonomous expert clinician working in a clinical specialist, whilst managing a clinical caseload of highly complex patients through expert physiotherapy assessment, treatment planning and discharge.

The post holder will be responsible for introducing new approaches to physiotherapy access and process increasing the focus on health promotion and self management.

The post holder will provide expert support and education for physiotherapists, health professionals, relatives and carers within and out with the service.

3.  ORGANISATIONAL POSITION

4.  SCOPE AND RANGE
·  Provides expert clinical skills in a specialist area across Lanarkshire.
·  To work at a very high organisational level of personal and professional autonomy and be able to make physiotherapy decisions where physiotherapy precedents may not have previously existed.
·  Is an authorised signatory for ordering small items.
·  Has responsibility for the development and implementation of evidence based assessment, treatment and discharge plans across all sites in collaboration with the multi-professional team.
·  To act as an expert clinical physiotherapy resource with advanced specialist knowledge and to provide education, direction and training to physiotherapy staff, other health care staff, GP’s, patient and carers both within the immediate service area and as a national resource.
·  To be responsible for and manage a highly complex patient case load where patients frequently present with conditions that have no medical solution and are associated with mental health problems, cognitive and/or behavioural problems, motivational and behaviour change difficulties.
·  To review and analyse current best practice and service provision in order to plan, develop and evolve the service.
·  To be responsible for, in conjunction with the care group lead, maintaining and improving all aspects of clinical governance, evidence based practice and quality assurance across the service.
·  In conjunction with the clinical lead be responsible for developing robust framework to monitor and evaluate clinical outcomes and standards of care.
·  Identify priority areas for leading research/audit, and promote and facilitate an environment to ensure integration of research evidence into practice.
·  Implementation of national and local guidelines in best practice in order to assist in driving divisional clinical development strategy within service.
·  To write, advise and provide support to other physiotherapists in the compilation of legal reports for solicitors in compensation and personal injury claims.
·  Reflection of own and current work practices to develop new and innovative ways of working including extending scope of practices.
5.  MAIN DUTIES/RESPONSIBILITIES
Clinical
·  To be professionally and legally responsible and accountable for all aspects of the own professional activities.
·  To assess capacity gain valid consent and have the ability to work within a legal framework with patients who lack capacity to consent.
·  To be responsible for and manage a caseload of patients with highly complex presentations.
·  To Work as an expert autonomous practitioner to examine assess and diagnose patients with highly complex presentations. Use expert knowledge, clinical reasoning skills and musculoskeletal expertise in conjunction with current evidence based practice to formulate diagnosis and prognosis allowing implementation of appropriate treatment plans. This process commonly includes patients presenting with unusual signs and symptoms where medical opinion may be conflicting.
·  To use expert manual therapy skills, that involves high levels of dexterity, co-ordination and palpatory skills for the assessment and treatment of some complex conditions. The expert skill is in the decision of which treatment approach to use.
·  Negotiate with patients on the return to activities where cognitive, behavioural and motivational issues may be significant barriers to change.
·  To use advanced theoretical and practical knowledge to refer to other health professionals as appropriate, in particular for further treatment or specialist consultant opinions and to ensure appropriate referral systems are used by departmental staff e.g. referral to clinical psychologist, orthopaedic service and community physical disability team.
·  To use expert assessment skills in the selection of patients for the further programmes of care (e.g. care management) and in the development of self management exit strategies for patients.
·  To act as an expert clinical physiotherapy resource with advanced specialist knowledge in the clinical area and to provide advanced direction and training to physiotherapy staff, other healthcare staff, GPs, patients and carers both within the immediate service area and as a national resource via special interest groups.
·  With the lead clinician continually develop robust frameworks to monitor and evaluate standards of care and clinical outcomes i.e. development / implementation of national clinical guidelines or benchmarking within similar specialities.
·  To be responsible for security and maintenance of accurate and up to date written records to include comprehensive progress and discharge reports to medical referrers and legal and disability reports, accurate mandatory statistical information to reflect care provided and meet professional, local and national standards.
Strategic/ & Leadership Responsibilities
·  In conjunction with the care group lead, continually develops the specialist physiotherapy service in line with Divisional and National strategic direction, incorporating patient/public involvement.
·  With the care group lead demonstrates professional leading by example and supporting staff in continual professional development.
·  To provide expert clinical supervision and be readily available to offer expertise to physiotherapy and other health care staff.
·  Contributes to the development of the wider physiotherapy service, identifying issues and pressures and initiating appropriate remedial action.
Planning/Operational Responsibilities
·  To take responsibility for the day-to-day management of the identified specialist service.
·  In conjunction with the care group lead has responsibility for a range of policy developments, within the service, and their implementation in conjunction with the multi-professional team.
·  To implement quality assurance initiatives within the clinical speciality, including clinical audits, clinical effectiveness projects and research.
·  To ensure the service complies with Professional, Organisation and National guidelines, procedures and policies.
·  To ensure audit, clinical research and quality programmes are supported and undertaken in the speciality.
·  To identify and implement service improvement.
·  In conjunction with the care group lead have responsibility for statistical analysis to monitor and evaluate the specialist service across Lanarkshire and to undertake service redesign.
·  To be actively involved with, and ensure that the service responds to national initiatives and policy within the framework of clinical governance including clinical effectiveness, evidence based healthcare, managing clinical risk and research and development issues.
·  In conjunction with the care group lead implements and monitors Health and Safety Procedures for the specialist service.
Educational
·  Facilitate increased awareness of evidence-based practice within the service and assist the organisation and development of specialist postgraduate education/ training of physiotherapists and other health care professionals working within the clinical speciality and other physiotherapy services.
·  To provide advice and act as an educational resource in the highly specialist area e.g. advising health care professionals on specialist techniques.
·  Responsible for developing the role of clinical specialist, including extension of practise and negotiating blurring of professional boundaries to aid more effective patient management.
·  Regularly deliver presentations in varying settings and varying specialist content dependent on the target audience. This may range from highly specialist training programmes (e.g. multi-professional and academic) to general information related to the clinical speciality (patient and public community settings).
6.  EQUIPMENT & MACHINERY
·  To understand, prescribe and ensure safe use of Physiotherapy Equipment; could include electrotherapy, exercise equipment, orthotics/appliances, prosthetics, mobility aids and assisted ventilation,
·  To understand and ensure safe use of additional patient care equipment; could include specialist beds, hoists, stand aids, sliding sheets and wheelchairs.
·  Maintain and ensure cleanliness as well as safety of equipment used for patient care in accordance with national and local policy
·  To be competent in the use of IT hardware and software utilised within the department; could include Word Processing, Spreadsheets, Databases, Presentation Software, E-mail, Internet, Physiotools, and Patient Management Systems.
·  To provide highly specialist advice in the selection, provision and use of all Physiotherapy Patient Care Equipment.
·  Authorise the ordering of equipment and provisions as delegated by the care group lead.
7.  SYSTEMS
·  Document patients records in accordance with professional standards to ensure continuity of care and compliance with current legislation. Records may be shared depending on patients need (e.g. other healthcare professionals, social work. lawyers etc)
·  Maintain a diary of patients appointments.
·  All records and I.T. systems will be maintained in accordance with data protection and I.T. security.
·  Completion and collation of personal timesheets and patients statistics for the specialist area as required by the care group lead.
·  Ensure that records are stored and maintained within physiotherapy standards and legal framework.
