SWYC Milestones Scoring Sheet

The SWYC Milestones is a 10-item questionnaire that parent’s fill out about their child’s development. The age ranges for the SWYC’s are:

  • 9 Months: 9-11 months
  • 12 Months: 12-14 months
  • 15Months: 15-17 months
  • 18 Months: 18-22 months
  • 24 Months: 23-28 months
  • 30 Months: 29-34 months

For each item, a parent can choose which of three answers best describes their child, in which:

  • Not Yet= 0 points
  • Somewhat= 1 point
  • Very Much= 2 points

1. To score, add up the total number of points. Any missing items count as 0 points.

2. Determine what specific age the child is (i.e. 11 months for the 9 month SWYC, etc.)

3. Refer to the score charts below.

4. Upon scoring the screen:

  • If a child is in the average or above average range, highlight the milestones that the child has reached.
  • If the child is below average, further investigation and referrals are recommended for any below-average scores.
  • Fill out an Early Intervention referral form and a Health Status Form and hand it to the clerical staff to fax to Early Intervention.
  • Briefly explain that you are referring the child for further developmental screening. Give parents the “Prescription for Early Intervention”.

The SWYC 9-Month Milestones screen can be given to children aged 9 months, 0 days to 11 months, 31 days.
