Job Description
Job Title: Support Worker
Name of Team: Outreach
Reporting to: Manager, Outreach
Job Purpose
To support clients to achieve their full potential. This will be achieved through working with clients in a range of settings in the community, including residents of the rehabilitation programme at Omnia who are aiming to move into independent living.
Core Responsibilities
Facilitate independent living through assistance in the maintenance and development of life skills and the building of resilience to mental ill health
Maintain positive and effective relationships with clients, other Norwich Mind staff, the staff of other agencies and the local community
Comply with Norwich Mind policies and Codes of Conduct
Specific Duties
Support clients in their mental and physical well-being, and report any concerns to the management team
Provide regular and intensive interventions when necessary
Advocate for clients, especially in the context of dealing with statutory agencies
Assist the management team in the development of new initiatives
Work with clients in such a way as to enable the provision of good quality guidance and support as appropriate to their needs, ensuring that client goals are reflected in care plans
Identify the changing needs of clients, and, as part of a team, provide an effective response
Respect the confidentiality of clients, staff and volunteers and to comply with the Data Protection Act
Ensure that clients are treated with a respectful and helpful attitude
Participate in team meetings
Undertake appropriate training and development and to participate in an annual appraisal with the line manager
Work in other Norwich Mind programmes when requested
Observe Health & Safety policies and report any concerns to the management team
Carry out any other reasonable duties as requested by the management team
