FORMATIVE ASSESSMENTS

Grades 1-3

  • In order to answer the questions in this template, you are to list the standards assessed in each formative assessment from one roster/preparation you submitted in Step 3 (either ELA or Math). You must also explain the scale used on these assessments.
  • Next, you are to select one of these formative assessments to discuss how the results of this assessment caused you to change/adjust your instructional practices.
  • At the end of this template, you should submit a blank copy of this formative assessment. In addition, you are required to submit a blank copy of one pre-assessment and a blank copy of one post- assessment for both Math and ELA.

A. List which standards were assessed by each of the formative assessments the preparation/subject and explain the meaning of the results you recorded on the roster (i.e. the scale used) For example, were the results recorded as percent correct, a particular level of a rubric, raw score, etc.

B. For the following questions, describe how you adjusted and delivered instruction based on student progress as evidenced by the results of the formative assessments.

Use the data, from the student roster corresponding to the blank formative assessment, you are submitting, to answer these questions. Identify the formative assessment to which you are referring (e.g. Math Formative Assessment #2) ______

Clearly explain how the results obtained from the formative assessment led to specific instructional changes.

1. Give specific examples of how you adjusted/changed plans and delivered instruction for whole groups (i.e. the entire class) based on the analysis of results from the selected formative assessment. What were areas needing intervention; how did your analysis of the results impact specific changes in instruction?

2. Give specific examples of how you adjusted/changed plans and delivered instruction for individuals or groups of students based on the analysis of their results. In what areas on the submitted assessment did the performance of individuals or groups indicate that an adjustment in instruction was needed? Explain what specific changes to instruction were made to differentiate for students whose performance demonstrated a lack of proficiency. Also explain what instructional changes were made for students whose performance on the assessment demonstrated proficiency.

You have now completed Step 4 of the Master Teacher application.

NOTE: You will submit the required documents listed above as part of your application.

NEXT APPLICATION STEP: (STEP 5)

In Step 5 you will select one student and discuss his/her performance on the formative assessment you submitted in Step 4. You are required to submit a copy of the student’s actual performance on this assessment.
