Intern Placement Tracking (IPT) Instructions for Agency Instructors

The Social Work Department utilizes an online system for completing field evaluations. Please follow the directions provided below completing midterm and end field evaluations on the student you are supervising. Logging in:

Changing your password:
1. Once logged in with the default username, you will be prompted to change your username and password. Please use your full name as your username. Example: Jane Doe
2. Set a unique password that you will remember. If you forget, click on “Forgot your username or password” at the login screen and your default username will be emailed to you. Jenny Becker can reset your access information as well.

- Change password: Allows you to modify your current password
- My Forms - As an agency instructor, you will complete a field assessment on the student you are supervising twice during the internship, at midterm and at the end of the placement. You will find the assessment under “my forms” once you log in. Typically, you will be notified of when to complete midterm and end evaluations by the faculty liaison.
- Midterm and End Evaluations (please utilize the assessment guide provided by Jenny Becker when completing the evaluation as the examples provided on that guide will be helpful when determining observable skills exhibited).
- Please indicate the time frame of the assessment (start date to mid term date).
- You will rate the student on the 9 social work competencies practice behaviors established by the Council on Social Work Education on a scale of 1-5. Only use NA if absolutely necessary and please provide comments if you do use NA. NA may be appropriate in the student has not yet had the opportunity to demonstrate the skill in agency setting, in which you can indicate in the comments section when that student will have an opportunity to demonstrate that skill. No NA should be used during the end evaluation.
- Please put comments under each core competency as it relates to the student.
- It is recommended that you complete the evaluation with your student or if you complete it by yourself, you share it with the student for feedback prior to the faculty liaison visit. The student may have some input on your ratings for behaviors you have not observed at the agency as some of the practice behaviors take place in the classroom.
- Once you, the student and faculty liaison have agreed upon the ratings (done at the midterm and end evaluation liaison visit), the student should check that they agree with the assessment, and all participants may click on their name. Once this is done, no revisions may be made to the assessment. If signatures need to be cleared for any reason, let Jenny Becker know.
- Print off a copy of the assessment for your records, if desired.
