Goal: To identify promising health workforce innovations that support delivery system transformation and develop recommendations for their implementation.
Objectives:
1.Review promising innovations in the use of health workers; this includes expanded scopes of practice and/or new roles for existing workers and new occupations in the context of health care teams, integrated delivery systems and increased coordination of care.
2.Review the evidence on the impact of these innovations on effectiveness, efficiency and quality of care.
3.Identify barriers to wide scale implementation; including regulatory, financial, cultural, organizational and educational; and
4.Make recommendations as to specific actions and strategies to remove barriers and to implement more widely. The recommendations will be based upon data to the greatest extent possible and on consensus of the participants.
This will be a working meeting. Participants will be expected to be actively engaged in reviewing the innovations and exploring next steps with white papers and publications to follow.

9:00 to 10:30: The Primary Care Team: New Roles and Relationships
As teams and medical homes have developed, numerous health systems have been exploring new relationships between primary and specialty physicians. In some cases, this has meant primary care practices taking on more responsibility with the help of specialists via telehealth. In other cases, it has meant building comprehensive health care teams including care coordinators and expanded roles for RNs taking on more management of patients with chronic illnesses. This session will explore what has been learned with recent projects.

There is a growing recognition that pharmacists can make an important contribution to improved care and outcomes, especially as part of a care team. There have been a number of demonstrations using pharmacists in an expanded role; others are exploring services that a pharmacist can provide consistent with their education and training.
