THIS IS THE 2015 REGISTRATION AND PAYMENT FORM
Please follow these directions to avoid mistakes with your registration:
- Fill in all the requested information in the fields marked with asterisks (*)
- Type the names of the participating students, exactly as their names must appear in the participation and award certificates.
- Type the name of the RESPONSIBLE TEACHER OR SCHOOL REPRESENTATIVE and date the form.
- Save the file as a word document: school name-registration 2015
- Send the document attached by e-mail to: before the deadline.
- Print the registration form, sign (responsible school person) and date.
- Write the check to: “SLU-Languages and Communication”.
- Mail the registration with the check to:

The following rules MUST BE observed in order to process your entry:
- TYPE the student’s name as you wish it to appear on the certificate. Be sure it is clear and legible. Complete all the requested information and sign the form.
- Enter only onename per individual category. For music: one group (up to 3 names), and for play: one group (up to 15 names + 2 substitutions). Leave the space blank if there is no entry.
- NO SUBSTITUTIONS / DELETIONS will be accepted after Friday, February 27th, 2015
- If a participant is not present for competition, no other student may serve as a substitute in the Poetry, Prose, Extemporaneous Speaking, and Music categories during the festival if the substitution was not processed on time.
If you plan to use alternates in the Play/Theater category, please list all the participants.
- Fee: $ 15.00for each participant per competition. Students are not allowed to participatewithout payment or registration.
- No late registration or payments will be accepted once the electronic registration is closed.
- The schedule of time/room slots for the competitions will be done on a first come first served basis. To avoid scheduling conflicts, please mark an asterisk on front of any name who is registered for more than one competition.
IMPORTANT NOTICE:
Due to the logistics of the festival,
No late registrations, deletions, or payments will be accepted after the electronic registration is closed. (No exceptions)
Since time and space for the Festival is limited, in the event that entry requests outnumber the maximum number of available time slots for a given category, the festival committee will make a selection to provide the widest possible participation by the greatest number of schools. Categories with less than three registered students will not be open for competition.
PLEASE PRINT / TYPE THE NAMES AS THEY HAVE TO APPEAR ON THE CERTIFICATES. PRINTING OF CERTIFICATES WILL NOT BE REPEATED DUE TO SPELLING MISTAKES.
ONLYONE ENTRY PER CATEGORY FOR INDIVIDUAL COMPETITIONS. ONLY ONE GROUP PER SCHOOL FOR MUSIC AND PLAY.

IMPORTANT: THE RESPONSIBLE SCHOOL REPRESENTATIVE OR LEAD TEACHER MUST READ AND SIGN THE STATEMENT BELOW. REGISTRATION WILL NOT BE PROCESSED WITHOUT THIS SIGNATURE.

- _____ No student whose name I have submitted for Poetry – Prose or Extemporaneous Speaking in a regular category is a native speaker of the language. Native speakers may only participate in Native Speaker competitions or in the Music or Play categories.
- _____ No student whose name I have submitted for regular categories is on a different level than the level of school years learning the language.
- _____ No student whose name I have submitted from a bilingual or a immersion school will participate on individual Tier 1 and Tier 2 competitions.
- _____ No students whose name I have submitted from a bilingual / immersion school is registered under the Tiers 1 or 2 for competition.
- _____ I have been informed that any category with less than three (3) students registered will not be open for competition. In this event, the festival will inform the school promptly, before the payment deadline.
- The festival coordinator will interact and respond only to the school representative whose signature is submitted below. Students and parents must not contact the festival coordinator.
By signing this statement the school I represent agrees to follow all of the rules and regulations governing the festival, to pay the fees for the total number of entries on the registration sheet, and to accept the final decisions of the judges and the competition scores.
