Ruocco summarized a recent Energy Daily article: Final Hanford Tank Cleanup Plan Draws State Concerns, which related to last month’s OEC Highlighted Presentation. Site Operating Experience Discussions
1. KSO / KCP
· At the Kansas City Plant, a sprinkler activated in a 70-year-old building and the cause is under investigation. Information will be shared when the investigation is complete.
2. Nevada Site Office (NSO) / NST / NSTec
· The site is submitting a LL to the DOE-HQ lessons learned database today about an event at the Joint Actinide Shock Physics Experimental Research facility (JASPER) where the contributing factors included overlooked/skipped procedural steps and lack of integration between the responsible managers. The lesson learned is that the cost of and time lost by going ahead and then having to rework is greater than the cost of waiting. Opportunities for Improvement were assigned.
3. Sandia Site Office (SS) / SNL
· An ORPS report was recently filed for a December 4 event when an experimenter - on an elevated test fixture platform - missed the second step when stepping off, fell to the concrete floor, and broke an elbow.
4. Thomas Jefferson Site Office
· Representatives discussed a recent event in which a vacuum cavity over-pressurized during tests being performed by a Graduate Student.The event demonstrated less-than-adequate student training and a lack of student understanding/awareness that once the apparatus is attached to the house nitrogen source, the apparatus becomes a pressure vessel. Inadequate supervision and procedural noncompliance are contributing factors. The event has spurred a review of training requirements. This information can be referenced in the DOE LL system under the following Title/ID: Insufficient Training and Supervision Lead to Cavity Window Failure, Lesson ID: JLabCOE376. This is not an isolated event – there have been similar viewport failures at SLAC and PPPL.
External Organizations Operating Experience and Lessons Learned Discussion
· Representatives of external organizations had no recent Lessons Learned to share.
· Hanford submitted a non-DOE lessons learned about malware/spyware that compromises SmartPhone applications, particularly those for Android. The Lessons Learned will be posted on the OE Wiki.

Operating Experience Initiatives
- ORPS Order and OE Order implementation- Ruocco
· EFCOG’s ORPS Task Group and HSS met at EFCOG’s ISM&QA Workshop at the end of November in Richland, WA, to work on DOE O 232.2, Occurrence Reporting and Processing of Operations Information, Responses to Frequently Asked Questions. Currently, the draft is going through HSS SME reviews.
· OE Wiki Video- the latest video posted was the “Turkey Fryer Fire Demonstration” from State Farm ®. To access the Video: http://operatingexperience.doe-hss.wikispaces.net/

· Lessons Learned Database improvements/upgrade will be assessed. HS-24 has been in contact with Hanford about their HILLS Program (internal Lesson Learned Database).

- Office of Analysis performance trending and analysis of ORPS- Skip Searfoss (HS-24)
· Skip presented the data and analysis performed on November ORPS reports including the number in each Significance Category, by PSO, and by HQ Keywords. The top two Keywords for November were Authorization Basis/12A (19%) and Injury/12H (11%). In fact, November had the highest number of AB events for any one-month period in the past two years. See more in the attachment posted on the OE Wiki.
- Self-Assessment reviews on the effectiveness of the OE Program- Ruocco
· HS-24 management is reviewing the results from HQ’s OE Self Assessment. A few of the outcomes of this self assessment include:
o Improve the Corporate Lessons Learned Database, and the quantity and quality of lessons learned submitted and maintained in the database.
o Formally verify existing and identify new Operating Experience (OE) Program Coordinators; ensure OE Coordinators are fulfilling their responsibilities and implementing OE requirements per DOE O 210.2A.
o Continue to improve the productivity of the OE Committee; the conduct and quality of monthly OE Committee meetings; and the engagement of appropriate site subject matter experts in sharing and discussing operating experience from site, national, and international sources.
o Improve the process to evaluate, publish, and disseminate operating experience from external sources (e.g., other Federal agencies; international sources).
o Verify that, as part of self-assessments conducted to evaluate organizational performance in ISM, Line Programs and contractors are including an assessment of the effectiveness of the organization’s operating experience program. Utilize the OE Committee in discussing lessons learned and areas needing improvement from these self-assessments.
o Continually improve performance trending and analysis of information from operating experience; develop more OE documents, to include a greater focus on nuclear safety.
· The OE initiatives listed above will be implemented throughout the coming year, 2013.
· Current Self-Assessments: Sara Langlais discussed NSTec’s OE Self Assessment that was recently completed. The Self Assessment was successful and improvements being made on their OE Program are mainly regarding communication. Langlais will send Ruocco their Self Assessment.
Who is conducting or has recently conducted a Self Assessment of their OE Program? If you have, please contact Ruocco and plan to share at the next OEC Call.
Feedback from the OEC is important in improving this Program. Please continue providing all feedback on improvements to the OE Program to Ruocco.
SPRING OPERATING EXPERIENCE COMMITTEE MEETING
The OEC plans to hold the OEC Conference during EFCOG’s Spring Meeting. It’s not too early to start creating an agenda and for you to start thinking about a short presentation you could give! Ruocco is requesting presenters, working group topics, discussion areas, and recommendations.
ACTION- OEC Members, please contact Ashley Ruocco as soon as possible if you would like to present at the Spring Meeting and if you have possible discussion topics.
