Music Information Form

Music copyright and licensing is a very important part of putting on a new piece of choreography, so we need to know something about the music you are going to use. Please fill in this form with your application; the easiest way is to do it on screen then send us the file, but you can print it out and send it by post, if you like.

How to use this form electronically:

·  Use space bar to tick/untick a box. You can also click on it to tick/untick.

·  Use the TAB key to move to the next field (and SHIFT+TAB to go back).

·  Start typing when the field is highlighted.

·  Save the form with your last name in the filename, e.g. <Smith Music Information-form.doc>

·  Either print or email the form back to us.

·  The music must not exceed 1 minute and 30 seconds. Any submitted music that exceeds 1 minute and 30 seconds will be rejected and a version of 1 minute and 30 seconds or under must be re-submitted within a week of notification.

·  The music can be submitted in either audio CD format, or by email, with MP3 files preferred.

·  Please note that only “topping and tailing edits” of recordings is permissible. There should be no “internal” cuts to the music as this may constitute an infringement of copyright.

·  All music will be trialled prior to the event.

All music submissions and files should be clearly titled, as follows:

·  Participant’s name (first, then last).

·  Name of their solo.


A completed copy of this form accompanied by a clearly marked exact recording of your music (with any cuts or fades already edited in) must be received by the RAD office no later than Friday, March 9, 2018. No exceptions. No music substitutions will be accepted once submitted. Please note: CDs and Mass Storage Devices will NOT be returned.
