Housing PrioritizationRegistry and Veteran By-Name-List Referral Workflow

The Alliance for Housing (Oakland County’s Continuum of Care) established a centralized Housing Prioritization Registry and a Veteran By-Name-List (BNL) to provide equitable and consistent access to all potential program participants seeking housing in our local community. Clients are referred to the registry/BNLif they meet one or more of the following:

Individuals and Households reporting chronic homelessness (regardless of VI Score) and qualifying disability
Individuals (not chronic) with a VI score of 8+ and qualifying disability
Families (not chronic) with a VI score of 9+ and qualifying disability
Youth (24 and under), regardless of VI Score or disability, who are Category I Homeless
Youth (24 and under), regardless of VI Score or disability, who are aging out of foster care, regardless of homeless definition(put note in HMIS regarding situation)
Veterans, regardless of VI Score or disability, who are Category 1 or 2 Homeless

Referrals will place the client on the Housing Prioritization Registry list and/or the Veteran’s BNL and will provide an opportunity for the client to be prioritized during the weekly Disposition Meeting.

Two workflows were developed to make referrals to the Housing Prioritization Registry and Veteran BNL:

Referrals to the Housing Prioritization Registry and Veteran BNL in HMIS

  1. Complete the assigned assessment and ROI based on your project workflow.
  1. Complete the VI-SPDAT for single clients and the F-VI-SPDAT for family heads of household.
  1. In all above instancesmake a referral to the Housing Prioritization Registryfor the head of household only.
  1. DO NOT BACKDATE. Click on the Service Transactions tab.
  1. Click on Add Referrals
  1. Under Needs Assignmentselect Supportive Housing Placement/Referral (BH-8500) and click on Add Terms(the selected need will populate at the bottom of the screen- see step #9). The Service Code Quicklist will vary depending on Codes assigned to your provider page.
  1. In the Referral Provider Quicklist section, select Coordinated Assessment – Oakland County – Housing Prioritization Registry (10285) and click Add Provider. The Referral Provider Quicklist will vary depending on providers assigned to your provider page.
  1. In the Referral Data section Search for the VI-SPDAT Score. Click on the green plus sign to select the VI-SPDAT assessment score. For the Family score (F-VI-SPDAT), record the score in the notes box in the Notes section at the bottom of the referral.

  1. In the Selected Needs Section the Supportive Housing Placement/Referral (BH-8500) should be selected. Select Identified for the need status, and leave the Outcome and If Not Met, Reason empty.
  1. In the notes section add in the Family VI-SPDAT (if applicable), the client’s sheltered or unsheltered status, the client’s phone number and/or email address, and name/organization of current case manager.
  1. At the bottom of the screen click Save ALL.
  1. The HMIS Administrator will run a referral report every Friday and add new referrals to the Housing Prioritization Registry and/or the Veteran BNL.

Referrals to theVeteran BNL Outside of HMIS

In instances where a user does not have access to HMIS, a referral to the Veteran BNL can be made directly to the HMIS Administrator.

  1. An intake is conducted with the Veteran participant and is eligible for Veteran Services.
  1. The case manager emails , through a secure email service (ex: zix), the following information:

Full Name
Date of Birth
Phone Number
Homeless Start Date
VI-Score (if completed)
Housing Plan Complete (y/n)
Agency Making Referral
  1. The HMIS Administrator adds the individual and information to the Veteran BNL.
