6. BRIFE RESUME OF THE INTENDED WORK:
6.1 Need for study:
The word “STREE” relates to her capacity of fertilization. The health of a woman to a large extent depends on the normalcy of her menstrual cycle.
In the present era being employed outside in a stressful atmosphere, the episodes of heavy menstrual bleeding are less tolerable, which disturbs her daily routine. The condition ends up with debility and psychological upsets, which hinders the achievement of conception.
As heavy menstrual bleeding (ASRGDARA)1 is a debilitating disorder, the woman requires rapid, safe and effective treatment. Ayurvedic texts have described a variety of treatment options in the management of “ASRGDARA”. Out of them the efficacy of “UTTARA VASTI” 1 is given much more importance by all the acharyas. Uttara vasti though a widely narrated therapeutic option is seldom in practice today. So now it is the need to evaluate the efficacy of this treatment modality scientifically.

· “UTTRA VASTI” has been explained in
Charaka samhita-siddisthana-9th chapter 1
Sushruta samhita-chikitsasthana-37, 38th chapter 2
Astanga sangraha-sootrasthana-28th, A.Hrdaya-sootrasthana-19th chapter 3
Previous works done:
· Jain Suman (1991); Banaras Hindu University-Varanasi, has done work on “Role of Yoga basti in Asrgdara”. 6
· Annathayya Sulochana (1992); Gujarat Ayurveda University –Jamnagar, has done a study of Asrgdara w.s.r. to DUB”. 6
· Kumari Ambili T(1996); Govt. Ayurveda college – Trivendrum, has done a study on the effect of Pushyanuga choorna in Asrgdarra.” 6
· Vinuta C (2003); SDMCA – Udupi, RGUHS Bangalore, has done work on a
clinical evaluation of the effect of certain indegenous drugs and Uttara vasti in Pradara w.s.r.to DUB”.
6.3) Objectives of the study:
· To make a comprehensive literary review of “ASRGDARA” and “DUB”.
· To evaluate the efficacy of “PUSHYANUGA CHOORNA” in Asrgdara.
· To evaluate the efficacy of “UTTARA VASTI” in Asrgdara.
· To evaluate the efficacy of “UTTARA VASTI ” after “YOGA VASTI ” in Asrgdara
· To compare the above 3 GROUPS.
7) MATERIALS AND METHODS:
7.1) Source of data:
Minimum 30 patients suffering from Asrgdara will be selected for the study from the OPD and IPD of S.D.M Ayurveda Hospital Kuthpady, Udupi.
7.2) Method of data collection:
· Patient presenting with complaints of excessive menstrual bleeding and irregular bleeding as that of menorrhagia, metrorrhagia and polymenorrhoea will be selected.5
· A detailed proforma will be prepared considering all the patients pertaining to history, signs, symptoms and examination as mentioned in our classics and allied sciences to confirm the diagnosis.
Inclusion criteria:
· Patient with age group of 20 to 45 years.
· Patient with pratyatma lakshana of Asrgdara.
· Patient diagnosed as DUB.5
Exclusion criteria:
· Patient with thyroid dysfunction, DM, HTN etc.
· Threatened or spontaneous or incomplete abortion.
· Bleeding disorders like thrombocytopenia etc.
· STD, Patient with IUCD, polyp, benign and malignant tumours of uterus.
INTERVENTION:
Patients fulfilling above criteria are selected and assigned in to 3 GROUPS :
GROUP 1 – of 10 patients will be administered “PUSHYANUGA CHOORNA” 4 about 12g with MADHU in three divided doses, with 100ml of TANDULODAKA in empty stomach for a period of one month.
GROUP 2 – of 10 patients will be administered “KASHMARYA KUTAJA GRITA UTTARA VASTI ” 1 for 3 days.
GROUP 3 – of 10 patients will be administered “KASHMARYA KUTAJA UTTARA VASTI ” 1 for 3 days, after a course of “YOGA VASTI ” for 5 days with “BALA TAILA ANUVASANA” and “DASHAMOOLA KASHAYA NIRUHA VASTI ”.
FOLLOW UP —Patient will be asked to follow-up for the next 2 cycles.
ASSESSMENT CRITERIA:
1.
· Duration of flow – if bleeding lasts for more than 5 days.
· Interval between 2 menstrual cycles – if more than 21 days.
· Amount of menstrual blood loss – use of no. of pads per day.
· Clots – present or absent.
· Staining – present or absent.
· Odour – present or absent.
· Pain – Mild (+)
Moderate (++)
Severe (+++)
2. Uterine size: - P/V examination or USG if needed
3. FINAL ASSESSMENT:
Cured
§ Regularization of menstruation, i.e. 3-5 days / 28-30 days.
§ Regularization in amount of bleeding, using 1-2 pads per day.
§ Reduction in associated symptoms like pain.
Improved - Mild improvement in amount of bleeding, length of the cycle & pain.
No relief – No change.
7.3) Does the study require any investigation or intervention to be conducted on patients or other humans or animals? If so, please describe briefly.
Yes, The study will be conducted on human subjects who match the inclusion criteria only.
Investigations:
Hb%, CT, BT, USG, Hormonal essay, Endometrial biopsy (if necessary).
