April 24-25, 2008, at the RadissonQuadCityPlaza. The city of Davenport was nationally recognized as the 2007 most livable small city in the country. CSET offers reduced rates to all active members.
In the very heart of the Midwest there’s a magical place nestled along the banks of the mighty Mississippi River. A place where two states are joined by the Mississippi to form the QuadCities, Davenport and Bettendorf in Iowa and Moline/East Moline and Rock Island in Illinois. This is your opportunity to convene with other professionals for camaraderie and to strengthen your knowledge in the challenging and rewarding fields of END and polysomnography.
Again this year we are offering two days of three courses each day with a variety of basic and advanced topics. These courses will fulfill continuing education credits.

All registration forms must be postmarked by April 16, 2008, to pre-register for this conference. We cannot guarantee space in the program or any program inclusions for those registrations we receive after this date.
A confirmation letter will be faxed or mailed if registration is completed through the mail by the registration deadline. Please note: Registration is not complete until you receive this confirmation letter.
You must bring this letter with you to the conference.
All cancellations must be in writing, via US mail and must be accompanied by the original registration receipt. Tuition for cancellations postmarked or date stamped before April 16, 2008, will be refunded less a $50.00 service charge. No refunds will be made after April 16, 2008
CSET reserves the right to cancel this program if sufficient registrations are not received. In case of cancellation, the society will fully refund registration fees.
