UNION MEMBER FACILITIES ACCESS MEMBERSHIP FORM
Purpose Statement: A collectively bargained benefit to promote the free use of the Montgomery County Department of Recreation’s gym/weight rooms and aquatic centers for all Fraternal Order of Police Montgomery County Lodge 35 active members.
Collective Bargaining Agreement, Article 29, Section G. All bargaining unit employees will be granted access to and use of recreation center gym/weight rooms and aquatic centers free of charge. In order to receive such access the bargaining unit members shall follow the administrative process established by the parties.
Agreed process:
· Complete the form below and mail/deliver to Montgomery County Department of Recreation, 4010 Randolph Road, Silver Spring, MD 20902; Attention: Registrar. The form can also be faxed to 240-777-6818.
· Fill out the form completely and include your ID number.
· Your information will be used to create an account for you.
· You will be notified when the account has been created.
· In order to use the fitness and aquatics facilities you must have an ACCESS card which will be scanned at each entry. You can obtain the ACCESS card at any Recreation Department facility. You must have this ACCESS card in order to use the designated facilities.
· Your ACCESS card will expire after one year and you must reregister in order to continue use of these facilities.
