This form is not necessary for SLO Data Submission as all teachers will submit this information through the “Student Learning Objective (SLO) Organization” on TSS. For your convenience, we have added notes that describe the information that you will need to collect and how you can expect to have to report it in TSS when you complete your SLO Data Submission. Feel free to print this worksheet and take notes on your planned SLO Data Submission for your own reference.

SLO Subject Area / Drop-Down / Select the option that best describes the subject area that you are including in your SLO Target Submission.
Options: CTE, Cognitive Development (for Early Learning only), Computer Technology, ELA, ESOL, Fine Arts (Performance Arts or Visual Arts), Health, Mathematics, Physical Education, Science, Social Studies, Special Education (only for Special Educators who intend to address multiple content areas in their SLO), World Language – French and Spanish, or World Language – non French and Spanish.
Course Duration / Drop-Down / Select the duration of the course(s) for which you will create your SLO.
Options: Full Year, Semester, Trimester
SLO Student Learning Target Group A / Free Text / In the space below, please state your specific SLO Student Learning Target A exactly as it is written in your “SY1617 SLO Target Submission” on TSS. You can copy and paste this language from #20 (“Target_A”) of your submission record from TSS.
Do not include data tables into this field. Only include your description of the SLO Target, all data will be submitted as an attachment to this form.
As a reminder, your SLO Student Learning Target for Group A is a description of success in the specific indicators of the SLO Learning Content and based on the assessment identified in your submitted SLO. This target may be a Growth Target or a Mastery Target, and should be sufficiently challenging for your Student Population based on the student baseline data and appropriate SLO guidance. Note: This describes success in terms of the level of achievement, and does not include the percentage or number of students who will reach the target.

You must update and attach this document to this Data Submission form. It will be used by your principal/supervisor to score your SLO based on the SY 16-17 SLO Final Scoring Rubric. This rubric is linked above for your reference. You may also upload a separate document that contains teacher comments as described above.
