Nomination Tip Sheet

Submission Requirements

Ensure your nomination contains all of the necessary documents. If your submission is incomplete, it will be reflected in your nominee’s final score.

Submissions must include:

·  A nomination letter: a brief biography and information about the nominee (two-page maximum). It must describe how your nominee meets the four criteria of the Cornerstone Awards (see below).

·  Exactly three (3) letters of support.

Nominations must be received by April 19, 2010 in order to be eligible.

1.  The Nomination Letter

Your nomination letter should describe how your candidate has met each of the four criteria.

In describing the nominee, focus should on the nominee’s attributes or successes that are not part of his or her day-to-day job requirements. For example, a hospital employee at the Information Desk is very good at giving directions to help patients and visitors find their way at the hospital. Although the person may perform this task exceptionally well, it is within the scope of his or her job.

Eligibility criteria:

Nominations should clearly demonstrate the leadership and skills that distinguish the individual or organization as exemplary.

·  Commitment to the values of the hospital, which are: respect, caring, innovation and accountability.

·  Commitment to health care that has exceeded normal expectations or has gone above and beyond professional duties in support of our hospitals.

·  Personal generosity or the degree to which an individual or group has inspired others with their spirit of philanthropy.

·  Leadership and innovation that have advanced health care at Hamilton

Health Sciences.

Examples of commitment might include:

·  Commitment to respect – a communication process that considers cultural diversity.

·  Commitment to caring - a person or group who has made a long-term volunteer commitment of time to help others or advance causes.

·  Commitment to innovation - a person or group who challenges the status quo to develop a new and better way of doing something.

·  Commitment to accountability - a person or group who is exceptionally honest, open and conscientious.

Tips for writing a good nomination:

·  Use a consistent, logical structure. Organize your thoughts: providing a summary first, followed by details. Use one point per paragraph, or short paragraphs.

·  Provide visual cues. Use fonts or underlining to highlight main points.

·  Use plain English – lay person’s language (please don’t use medical terminology.)
