The members of the ECCT are bound by their professional codes of ethics to treat any personal information as confidential.

The Bute ECCT work as an integrated team which means we work with both health and social work. This means that sometimes we need to pass on some information about you to another service in order to meet your needs.

The detail of what can be shared and with whom is explained and agreed with you before information is passed on.

WHAT ABOUT EQUIPMENT?

You will need to be assessed before equipment is issued for the first time, but if something is faulty or you wish to return equipment that is no longer needed please contact 01700 501541

Bute Extended Community Care Team (ECCT)

WHAT IS THE EXTENDED COMMUNITY CARE TEAM?

It is a joint health and social care team and includes the following teams:

Occupational Therapy

Physiotherapy

Community Nurses

Social Work

Health and Social care assistants

The team works closely with other specialist services as required.

AIMS OF THE EXTENDED COMMUNITY CARE TEAM

-  To support people who have health/social needs to live at home

-  Prevent unnecessary hospital admission

-  Support / facilitate hospital discharges.

-  To maintain independence and quality of life

REFERRAL TO THE EXTENDED COMMUNITY CARE TEAM

We accept referrals from individuals, family, friends and health and social care professionals. Please use the contact number, email, or postal address overleaf to make a referral.

Please note that health, social work, and other professionals should fully complete the ECCT referral form before a referral can be accepted.

A referral to the ECCT is appropriate for an individual who, due to changes in home circumstances and /or physical abilities, requires enhanced community services support to remain at home.

This can be achieved through;

-  Nursing assessment and treatment

-  Occupational therapy assessment and intervention

-  Physiotherapy assessment and intervention

-  Provision of equipment and adaptations

-  Telehealthcare

-  Referral to voluntary organisations

-  Care at home services

-  Specialist services

-  Reablement services

Referrals to the ECCT are discussed daily at a multi –agency meeting and intervention is agreed.

WHO ARE WE AND WHAT DO WE DO?

Community Nursing

Community nurses work is carried out primarily in the patient’s home. Our work involves:

-  care of patients with long term conditions

-  Palliative/ end of life care

-  Self management

-  Post-operative care

-  Assessment and management of wounds

-  Continence advice

-  Pulmonary rehab and telehealth

Occupational Therapy

OT is the assessment and treatment of people with any kind of illness, injury or disability to promote independence and enhance quality of life. We can recommend a reablement package or provision of equipment and adaptations.

Physiotherapy

The physiotherapist can offer intervention that helps to improve mobility and aids in the prevention of falls. Advice and guidance on an appropriate exercise programme may be included as part of your treatment.

Reablement

Reablement is short term support to help you back to independence. It is about working together on the things that are important to you to enable you to manage your everyday life as independently as possible.

Social Work

Provide or purchase a wide range of services to support people to live as independently as possible within their own home or in a homely environment. Our services include; support at home services, carers assessment, meals on wheels, telecare, day services, respite services, and residential/nursing care.
