PART A. The organization has to prepare the application and do a self evaluation prior to submitting this application:
  1. Complete 2 x original signed copies of this application
  2. Attach 2 x certified copies of the modular logbook hereto (100% of the codes / grid signed by the apprentice and designated mentor)
  3. Attach 2 x certified copies of the national ID
  4. Attach 2 x certified copies of a valid N2 certificate with Mathematics, Relevant Trade Theory, plus two relevant subjects
  5. Attach 2 x certified copies of the recommendation and motivation issued by an accredited provider for exemption of N2 if applicable
  6. Attach 2 x originally signed lettersof conformation on company letterhead
  1. Declaration that the apprentice had been/are employed and received workplace exposure at the company under mentorship of a qualified artisan for the designated trade
  2. Confirmation that the apprentices completed all the practical and theoretical training in accordance with the MERSETA training schedule for the designated trade
  3. Recommendation that the apprentice is trade test ready
  4. Declaration that the apprentice completed a minimum period 85 weeks practical workplace exposure during the contractual period of the apprenticeship
  1. Attach 2 x certified copies of previous trade test results (NYC declaration)
  2. Attach proof of payment of trade test fee at the designated Trade Test Centre
