These following statements are about your experience as a parent of a young child/ young children. For these statements, please mark the answer that best describes your level of agreement with an “X”.

- Place the envelope in the box at the front of the room along with the sheet you tore off.
FYI, later during the program, you will get this sheet back so you can answer the statements on the back.
