GENERAL INSTRUCTIONS:

1.  Using the three accompanying printouts, complete the all items on this questionnaire. These questions are intended to walk you through the interpretation of a multi-informant assessment using the BASC Teacher Rating Scales (TRS) and Parent Rating Scales (PRS).

2.  After completing this questionnaire, write an interpretive summary of these assessment data as it might appear in a psychological evaluation report. Summaries are not to exceed one page and must be typed and double spaced. Refer to the sample report provided in class for general guidance regarding the summary. Summit your summaries on a separate sheet, attached to this questionnaire.
