What is the Core Data Form?
The Core Data Form (CDF) collects core data about the people, businesses, and organizations that are regulated by TCEQ.
When do I complete this form?
A Core Data Form should be submitted under the following circumstances:
- You do not have a CN and/or RN for any program with the TCEQ. If you are regulated by the agency under another permit or program, you most likely have a CN and RN.
- Your current ownership or location information has changed.
- You need a TXT2 Number.
- It is requested by the Agency.
You can view/review your data in Central Registry if you are regulated by programs other than the Tier II Program. Due to sensitive information security, Tier II data willNOTbe visible to the public. Use this web link to see non Tier II data:

What is a Customer?
Customer - An Individual or Organization responsible for one or more Regulated Entities. Examples include owners, operators, or responsible parties of a site or an individual who holds a license. Note: The Customer Name submitted may be updated automatically based on what is current and active with the Texas Secretary of State (SOS) or Texas Comptroller of Public Accounts (CPA).
Customer Reference Numbers (CNs) are used to identify Customers in Central Registry. Each Individual or Organization is issued an unique 11-digit identification number called a CN (e.g. CN123456789). Each Customer should have only one CN based upon their name and SOS/CPA information as applicable.
What is a Regulated Entity?
Regulated Entity – This is the site that’s being regulated by TCEQ. The Regulated Entity (RE) is located at the actual physical address where the regulated activity is occurring. Examples are a site or facility. Note: The Regulated Entity Name submitted may be updated in order to meet TCEQ Agency Data Standards (removal of organizational endings such as Inc, LP, or LLC etc. from the Regulated Entity name).
Each site location is issued an unique 11-digit identification number called a RN (e.g. RN123456789). Each Regulated Entity should have only one RN number unless their activity is in a program with a stand-alone designation.
*The CN and/or RN will not replace program-specific numbers (e.g. Permit, Registration, Account, etc.)

Do I receive a new CN or RN Number each time I submit a CDF?
No. A new CN or RN number is assigned only if:
- The Customer does not have a CN in Central Registry
A Customer may be involved with many sites but should only be issued a single CN in Central Registry based upon the Individual’s Name or Organization’s SOS/CPA information as applicable.
- The Regulated Entity does not have a RN in Central Registry
A Regulated Entity identifies where the regulated activity is occurring. There should be only one RN number issued for each location unless the activity is in a program with a stand-alone designation. The RN will stay with that site/equipment regardless of how many times the responsible Customer changes.

Leave this field blank if the specific site does not have an RN, the TCEQ will issue RN number(s) .If this location has been issued a RN, enter it here.
Section II: Customer Information
4. General Customer Information
Check ALL boxes that apply.
- New Customer
The Customer does not have a CN. Complete all items in Section II.
- Update to Customer Information
The Customer has a CN but some of the
information in Section II has changed and needs to be updated. Complete the items that have changed.
- Change in Regulated Entity Ownership
The Customer for this Regulated Entity (listed in field 3) has changed (e.g. Facility has been sold or is now under operational control of another Customer). Complete all items in Section II and fill in the previous Customer’s name and effective date.
- Change in Legal Name
A Customer name change will require verification with the SOS/CPA prior to it being processed by the Agency.
5. Effective Date
Enter the effective date of the update (e.g. date of sale, SOS filing date, etc.).
6. Customer Legal Name
Enter the complete legal name of the Customer.
Verify the legal name of your organization through the SOS and CPA databases.

Enter the prior name of the Customer if the Customer name is changing due to a change in the legal name.

7. TX SOS/CPA Filing Number

8. TX State Tax ID

9. Federal Tax ID
All businesses, except some small sole proprietors and individuals should have a federal taxpayer identification number (FEIN). Enter it here. Do NOT list hyphens, dashes, or a Social Security Number.
10. DUNS Number
ENTER your Data Universal Numbering System (DUNS) number.

Leave this field blank if a DUNS number has not been issued by Dun and Bradstreet Corp (usually larger companies). If aTier II report has been submitted previously, this may be found in your report under the ID and Regs tab.
11. Type of Customer
Check only ONE box.
- Corporation
The Customer meets all of the following:
- Is legally incorporated under the laws of any state or country
- Is recognized as a corporation by the Texas Secretary of State
- Has proper operating authority to operate in Texas

- Subchapter S corporations
- Limited liability companies (LLC)
- Water supply corporations
- Individual
An individual is a person who is regulated by TCEQ, but has not established a business.
- Partnership
- General Partnership - A general partnership is created when two or more persons associate to carry on a business for profit. A partnership generally operates in accordance with a partnership agreement, but there is no requirement that the agreement be in writing and no state-filing requirement. Please submit a tax ID if you have a sales tax permit.
- Limited Partnership (LP & LLP) - A Texas limited partnership is a partnership formed by two or more persons, having one or more general partners and one or more limited partners. The limited partnership operates in accordance with a partnership agreement, written or oral, of the partners as to the affairs of the limited partnership and the conduct of its business. While the partnership agreement is not filed for public record, the limited partnership must file a certificate of limited partnership with the Texas SOS. They provide a form for the certificate of limited partnership which meets minimum state law requirements.
- Government – City, County, Federal, or State
The Customer is either an agency of one of these levels of government or the governmental body itself (e.g. Blanco County, City of Houston, Texas Department of Transportation, etc.).
- Government – Other
The Customer is a utility district, water district, tribal government, college district, council of governments or river authority (e.g. Lower Colorado River Authority).
- Sole-Proprietorship
The Customer is an individual who owns or runs an unincorporated business with no distinction between the business and the owner.
- Other
The Customer does not fit any of the descriptions above (e.g. joint venture, trust, estate, etc.). Enter a short description in the space provided.
12. Number of Employees
Enter the number of employees for the Customer’s entire company, including subsidiaries.
13. Independently Owned and Operated
Check “No” if the Customer is a subsidiary or part of a larger company. Otherwise, check “Yes”.
14. Customer Role
Check ONE box that best indicates the Customer’s relationship to the Regulated Entity. Check “Other” if none of the boxes fit your situation and write in the relationship.
15. Mailing Address
Enter the mailing address where general correspondence will be received regarding Customer information (e.g. the Customer’s Headquarters). This should be a deliverable address recognized by the United States Postal Service. Use this web link for address verification:
Enter the mailing address for the overall/primary Customer contact for TCEQ. Do NOT enter specific program contacts (e.g. New Source Review Contact, Petroleum Storage Contact, etc.).
16. Country Mailing Information
Leave this field blank if the address is in the USA.
17-20. E-mail, Phone and Fax Information
Enter the communication information that TCEQ will need to contact the Customer. Leave field 19 blank if your phone system does not use extensions or codes.
Section III: Regulated Entity Information
21. General Regulated Entity Information
Check ONE box
- New Regulated Entity
The Regulated Entity does not have a RN. Complete all items in Section III.
- Update to Regulated Entity Name
The known name of the Regulated Entity has changed and needs to be updated. This name will be used by all program areas.
- Update to Regulated Entity Information
The Regulated Entity has a RN but some of the
information in Section III has changed and needs to be updated. Complete the items that have changed.
22. Regulated Entity Name
Enter the name the Regulated Entity will be known by. It will be used by all TCEQ programs when referring to the Regulated Entity. Make sure this name is consistent on all forms submitted to TCEQ.
Note: The Regulated Entity Name submitted may be slightly amended in order to meet TCEQ Agency Data Standards (removal of organizational endings such as Inc, LP, or LLC, etc. from the Regulated Entity name).
23. Street Address of the Regulated Entity
Enter the physical street address of the Regulated Entity. Do not provide a P.O. Box. If there is no street address, complete fields 25 and 26.

24. County
Enter the county where the Regulated Entity is located. If there is more than one county, enter the primary county in which the majority of the Regulated Entity is located.
25. Description to Physical Location
If the physical location has no street address, enter specific directions of how to reach the Regulated Entity.
Example:FROM US 83, GO E ON COUNTY RD 101.GO 0.3 MI PAST FIRST TANK ON LEFT.
26. Nearest City, State, and Nearest ZIP
If the Regulated Entity is in a city, that city will be the nearest city. If the nearest city is in another state or country, please provide that information in this blank as well
State: This will primarily be Texas.
Nearest ZIP Code: Enter the ZIP code for the area where the Regulated Entity is located. If not available, give the ZIP code that is closest to this Regulated Entity.
27-28. Latitude (N)and Longitude (W)
Enter the latitude and longitude for a fixed location, if known. For larger sites use the center point of the operation. It can be entered in either decimal or degree format. If you have previously completed a Tier II report, this will be available under the location tab in the software.
29-32 SIC and NAICS Codes
List your SIC or NAICS code. If you have previously submitted a Tier II report, it will be under the ID and Regs tab of the software.

33.Primary Business of this Entity
Briefly describe the primary business of this Regulated Entity (e.g. Gas Compression, Fleet Refueling, Rubber Manufacturing, etc.). Do not repeat the SIC or NAICS description.
34. Mailing Address
Enter the mailing address where general correspondence will be received (e.g. the Main Office). This should be a deliverable address recognized by the United States Postal Service. Use this web link for address verification:
35-38. E-mail, Phone and Fax information
Enter the communication information that TCEQ will need to contact the Regulated Entity. Leave Field 37 blank if your phone system does not use extensions or codes.
39. TCEQ Programs and ID Numbers
Use these fields to indicate the Programs and Permit/Registrations that will be impacted by the updates noted on this form. Select “other”, enterTier II as the program name and enter your TXT2 number below the program name if you have one.
Section IV: Preparer Information

Enter information for the person who actually completed the form. This person will be contacted if TCEQ staff has questions.
