- IMPORTANT INFORMATION:
1.2Depending on the number of years registered with the club, other players may be asked to provide photographs as well. Player cards need to be produced and maintained and without cards players will not be allowed to play in matches. Cards are the property of ELFA and must be returned at the end of season or when a player leaves the Club.
1.3The annual Club membership fee is R1700 for the first child, R1500 for the second child and more children (of the same family).The fee includes a kit.
1.4Payment can be made electronically by depositing the required amount in the following account:

1.5Please use the child’s surname, initial and age group as payment reference, for example: JallalMU13.
1.6Proof of payment must be emailed or faxed to the Team Manager.
1.7Players will not be allowed to play in league or other matches unless the registration form and the indemnity form have been received by the Club and all fees are paid, proof of payment received, or arrangements have been made with the committee.
1.8Kit will be handed to the Team Managers when available, provided payment has been received.

INDEMNITY AND CONSENT CONFIRMATION

1.Do hereby indemnify and hold harmless the Club (Benoni All Stars Football Club) and each of its officers and employees against any claim, damage, injury, and/or loss which I and/or my child may sustain by virtue of any conduct and/or sports injury which may arise whilst my aforementioned child participates in sport for and on behalf of the abovementioned Club.
2.I further agree to pay as liquidated damages to the Club the amount of money imposed on the
Club as a fine (or such pro-rata share as the Club Committee may determine), by any competent body which shall have the authority to impose such fine as a result of the misconduct of either myself and/or my child or any other member of my family, of whose misconduct I acknowledge the Club Committee to be the sole and absolute judge.
3.I also understand that notwithstanding the fact that I have registered my child with (Benoni All Stars Football Club), the Club cannot guarantee that my child will play a match each week.
4.I consent to personal information of me and my child being used for registration purposes. I also consent to the use of photographs of me, my child in his/her capacity as (Benoni All Stars Football Club) member and related to soccer, being used by (Benoni All Stars Football Club) on electronic and other platforms.

PARENT / GAURDIAN CODE OF CONDUCT
The following standards are established by Benoni All Stars Football Club and govern the behaviour of all BAS Parents and spectators before, during and after soccer club matches, at a home or away game:
As a requirement to register my child to participate in any Benoni All Stars Football Club Junior Soccer season
I therefore agree:
1.I will not force my child to participate in sports against their will.
2.I will remember that children participate to have fun and that the game is for them, not for me
3.I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.
4.I will learn the rules of the game and the policies of the league.
5.I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice or tournament that BAS is involved in.
6.I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures.
7.I will not encourage any behaviours or practices that would endanger the health and emotional well-being of the players.
8.I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence.
9.I will demand that my child treat other players, coaches, officials and spectators with respect regardless of race, creed, colour, sex or ability.
10.I will teach my child that doing one's best is more important than winning, so that my child will never feel defeated by the outcome of a game or his/her performance.
11.I will praise my child for competing fairly and trying hard, and make my child feel like a winner every time.
12.I will never ridicule or yell at my child or other participant for making a mistake or losing a competition.
13.I will emphasize skill development and practices and how they benefit my child over winning. I will also de-emphasize games and competition in the lower age groups.
14.I will promote the emotional and physical well-being of the players ahead of any personal desire I may have for my child to win.
15.I will respect the officials and their authority during games and will never question, discuss, or confront coaches at the game field, and will take time to speak with coaches at an agreed upon time and place.
16.I will demand a sports environment for my child that is free from drugs and alcohol and I will refrain from their use at all sports events.
17.I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches of the team or asked to fill in, by the coach.
18.I recognize that COACHES ARE VOLUNTEERS. They give up their time for my child. I will SUPPORT them holistically.
19.I will always be proud of being a part of Benoni All Stars Football Club and uphold its name as a representative.
STADIUM RULES.
- ONLY PLAYERS AND COACHES WITH CARDS WILL BE ALLOWED ON THE COACHES SIDE OF THE FIELD.
- ALL PARENTS AND SPECTATORS WILL BE BEHIND THE FIELD MARKERS.
- NO GLASS BOTTLES ARE ALLOWED ON THE FIELD.
- THE STADIUM IS A NO-ALCOHOL ZONE.NO ALCOHOL ALLOWED WHAT SO EVER.
By registering your child to participate in any Benoni All Stars Football Club Junior Soccer Season, you also agree that if you fail to abide by the aforementioned rules and guidelines, the parent/guardian will be subject to disciplinary action by the Committee of BAS, which could lead to eventual forfeiture of the right of participation. I have read, understood and accept the conduct expected of me as outlined above.
By signing and dating you agree that you are bound by this agreement and understand that I must remember how my team behaves is a reflection on me. As a principle, good conduct and mannerism, as anticipated by a reasonable person, on an off the field is agreed to.
