The following questionnaire has been prepared to collect information on municipal waste collection, recycling, composting and disposal from all non-unitary Waste Disposal Authorities (WDAs) in England. This information is required to formulate and implement waste management policies effectively, to monitor progress towards national and local targets and to enable the UK to meet international reporting obligations. Separate questionnaires have been sent to all non-unitary Waste Collection Authorities (WCAs) in England and Unitary Authorities (UAs) in England not covered by a joint Waste Disposal Authority.
When completing the questionnaire please note:
• all data relate to the financial year 1 April 2003 to 31 March 2004, referred to throughout as 2003/2004;
• financial information is not required;
• if you are unable to complete all the questions, please complete as much as possible and return the partially completed questionnaire.
You may need to consult a number of officers in your authority to provide accurate information. We would be grateful if one officer would take responsibility for co-ordinating the response and for returning the completed questionnaire. This officer should be nominated as the contact point in Section D. If at any time this officer should change, please let us know so that records can be updated.

Conventions
All quantities should be in TONNES, including data on oil.
1000 litres of oil = 0.9 tonnes.
If you are unable to convert individual units to tonnes, then please seek advice on how to do this from the consultants (contact details on the front page).
When filling out the questionnaire please use the following conventions so that you do not leave blank or empty cells (except where you are asked to skip whole sections):
1) 0 (zero) to denote nil.
2) N/A to denote not applicable.
3) – (dash) to denote data not available.
4) Where data is included in the answer to another question, please state which question, for example "Data is included in the answer to question 4.2".
5) Please put any additional notes to any question in Section D1 at the end, and indicate next to the question(s) concerned that you have done this.
6) When completing questions which require a tick in a box, please do not use any symbols other than a tick.

From Section B
11 ‘Private waste disposal contractor’ is used here in accordance with the definition in Environmental Protection Act 1990. It therefore includes those providing a recycling service and private managing agents.
12 Include landfill, regardless of whether methane is recovered, and incineration without energy recovery.
13 MRF means Materials Reclamation Facility.
14 Include composting operations and incineration with energy recovery here.
15 As in Schedule 1 of the Controlled Waste Regulations 1992, although it is acknowledged that there may be some commercial or industrial waste included in the totals.
16 As in Schedule 2 of the Controlled Waste Regulations 1992 and will include such services as street sweeping, bulky waste collection, hazardous household waste collection, litter collections, household clinical waste collection and separate garden waste collection (not for composting). The wastes covered should correspond to those covered by question A1.4.

18 ‘Mechanised metal extraction’ covers machinery for extraction of ferrous or non-ferrous metals. It does not include hand sorting.
19 Include waste sent indirectly to landfill via techniques such as baling, shredding or via transfer stations.
20 Exclude waste sent for recycling, composting or sent to transfer stations. This waste should be included under Ultimate treatment and disposal options – these are covered in Part C.
Exclude wastes that may be processed prior to landfill, for example by baling or shredding – these should be included in the landfill total.
Exclude wastes sent to Materials Reclamation Facilities (MRFs) – these are covered in Part C.
Include wastes sent for anaerobic digestion.

24 Please ensure units are converted into tonnes. For oil, the conversion rate to be used is 1000 litres = 0.9 tonnes.
25 ‘Co-mingled materials’ are waste collected in a mixed form that is destined for recycling after further sorting. Avoid using this entry if at all possible. Only use it if you are unable to obtain a breakdown of co-mingled materials from, for example, a MRF which processes them.
26 Not applicable to WDA questionnaire.
27 ‘Civic amenity site’ means a location provided by the local authority, where the public can deliver their waste, including bulky items such as beds, cookers and garden waste as well as recyclables.
