______* I agree to have the dog spayed/neutered in the month listed above. All puppies/dogs adopted from the MSPCA must be spayed/neutered. An MSPCA member will phone you to assist in arranging the veterinary appointment**.

1.  I understand the dog is being re-homed to me as a house pet/companion and will be treated as a family member.

2.  The dog will live and sleep inside my home; and will not be kept closed in a kennel or shed. The dog will not be chained or tied up outside.

3.  I will provide a securely fenced yard/garden and keep the dog under proper control at all times.

4.  I will ensure the dog is always wearing a collar with an ID tag that includes my name and contact numbers.

5.  I will ensure to keep current the dog’s microchip details with his/her registered database (i.e., FIDO/Pet Log).

6.  I will provide qualified veterinary treatment upon sickness or injury; and keep current all annual vaccinations.

7.  I will ensure the dog is licenced and abide by all applicable laws.

8.  I will notify the MSPCA, Dog Warden, Pound, Garda, and local shelters should the dog become lost or stolen.

9.  I will return the dog to the MSPCA, if I am unable to care or keep the dog. I will not give away, sell, or dispose of the dog in any other way.

10.  I have accepted possession of and responsibility for the dog identified above and thus release the MSPCA from any liability, claims or damages.

11.  I understand that the MSPCA provides as much information & history as possible; they cannot guarantee the health, behaviour, age or temperament of the dog.

12.  I understand that the fee/donation paid is not for the purchase of the dog, but also for the care and veterinary costs to help current and future MSPCA rescue dogs.

13.  I understand the MSPCA has the right to do a home check to ensure the home and enclosure area are suitable.

14.  I understand the MSPCA reserves the right to reclaim the dog if they feel he/she is not being properly fed, housed or cared for to their satisfaction.

I understand that by voluntarily signing this Agreement, I am entering into a legal and binding Contract with the MSPCA, and I agree to ALL the above conditions of adoption.

The MSPCA acknowledges receipt this day, from the adopter named above, a ______adoption fee/donation.

** The MSPCA will contribute (€75 - £75) for spaying and (€50 - £50) for neutering a dog re-homed from our shelter.
