APPENDIX R: Facilitator Checklist
Program preparation
Propose possible dates for program sessions with venue, partners, coordinator, and possible guest speakers.
Investigate possible program venues and visit sites to ensure adequate space and equipment needed.
Find guest speakers to assist in presenting modules by connecting with community partners.
Recruitment of participants: Distribute flyers at local grocery and health food stores, local markets, non-profits, community organizations, faith groups, etc.
Confirm venue.
Purchase participant binders and page dividers.
Purchase or gather other materials needed for presentations: flip chart, markers, paper, stapler, hole puncher, post-it notes, projector, screen, extension cord, pens, etc.
Confirm dates with participants and number of confirmed attendees.
Discuss and confirm modules to be presented.
Confirm guest speakers and equipment they will need to present.
Assemble participant binders.
Plan daily agendas including menu choices for snack and lunch. Remember to consider preparation time and cooking time for each recipe.
Create a grocery list and a kitchen equipment list for all recipe choices.
Check equipment inventory of onsite kitchen and work space.
Print 7 core modules and recipes for participants.
Collect participant’s contact information (phone number, address, e-mail).
Purchase guest speaker “thank you” gifts and cards if applicable.
Connect with facility manager to confirm time of arrival.
Print resources for participants before each session.
Adjust recipes to reflect number of servings needed.
Purchase groceries day before workshop.
Print a master copy of your daily recipes; place them in a plastic page protector for use during workshop days. It is helpful to tape the recipe to the wall above your kitchen work space.
Collect cooking supplies, equipment necessary to complete recipes.
Confirm all audio visual equipment is available and functioning.
Prepare name tags
Review toolkit information, facilitator tips and mental fitness information
Program delivery
Collect sharing of information consent forms from participants (APPENDIX H).
Distributepre-program questionnaire (APPENDIX J).
Welcome participants, reflect on previous session, and discuss the agenda for the day.
Arrive 1 hour early to set up tables, handouts and kitchen space.
Plan for cleanup of the kitchen and work room areas.
Post-program
Answer coordinator questionnaire (APPENDIX I).
Distribute and collect evaluations from participants on final day (APPENDIX K).
Return all pre and post workshop evaluations to provincial Community Food Mentor Advisory Committee.
Save Community Food Mentor’s contact information and share with Community Food Mentor Advisory Committee.
Develop plan of ongoing communication between CFMs.
Develop plan to continue supporting work of CFMs.
