4th Corporation implements policies and procedures that protect from ANE and mistreatment / Policy specifically details definitions of ANE as defined by ND Century Code.
Yearly training on reporting and definitions of ANE and serious events. /
- Continue General Events reporting via Therap. All staff have been trained in RDG’s and reporting protocol.
- Participate in audit review of GER’s
- Follow up on GER ‘s will be detailed in Therap system. Update these at HSW when review occurs.

4th Corporation advocates for best possible health. / At admission people are given the opportunity to choose a doctor. This is reviewed annually at the PCSP and changes are made if desired. Screening scheduled as recommended by CDC and WHO. /
- Continuethe Health Services Satisfaction Survey
- Continue to promote Infection Control practices within the agency. Continue to offer free Hep B vaccine to employees.
- Medical department ALERTS and reminders for appointments.
2.Flu clinic offered at the center. Infection control inservice done yearly. Discussed monthly at HSW meeting.
3.Alerts sent out for staff to see and address changes. Reminders sent out for appointments and to allow people to check their calendars to make sure the time is convenient for them.
4th Corporation provides safe environments. / The Emergency and Disaster Preparedness Policy has been updated to include severe weather, flooding and other natural disasters.Continue partnership with workforce safety on the Safe Management Program, Safe Lift Program, Safety Committee Program, and Drug Free Workplace /

- Continue to update homes with fire safety equipment.
- Continue to look at population of the homes and the needs of the people who live there.
- Staff will continue to be aware of community resources in the event of disasters.

- Annunciator panels added to fire alarm systems. Continue to save funds so sprinkler systems can be put in.
- Renovations at the homes occurring to make bathrooms more accessible.

- Yearly training on 4th Corporations Safety Policy that includes mandatory training on back safety.
4th Corporation promotes staff resources and supports. / Satisfaction Survey is sent out each year to all staff. Continue to collaborate with Minot State University on the training program which is now credentialed through NADSP. Continue to evaluate employees using peer input using a 360 degree process. /

4th Corporation strives for positive services and supports. / Continue using the Positive Behavior Supports module as a requirement of certification. /
- Continue to collaborate with Behavioral specialist to ensure plans are done in the least restrictive way.
- Continue to collaborate with outside services to ensure people can maintain their quality of life (Lake Region Human Service center to use counselor , Life Skills transition center CARES team)
- Emphasize Positive Behavioral Supports in training.
- Continue to work with the Transition and Diversion Coordinator through the DD Department.
- Chart PRN usage data. Followup by behavioral specialist with the people who work directly with the person to reduce the usage.
- Schedule the counselor in every other week for those who wish to see her. Request use of CARES team when needed.
- Enhance the Therapeutic Responses through Positive Behavioral supports class to focus more on PBS when dealing with challenging behaviors.
- Monthly updates provided to the Coordinator on issues within 4th Corporation.
4th Corporation monitors Basic Assurances. / 4th Corporation’s Strategic Plan is updated yearly. /
- The Leadership Committee will monitor Basic Assurances.
- Teams will review Basic Assurances at the annual PCSP for each person
- Complete the Basic Assurances Self Assessment yearly.
- This has been incorporated into the Agenda for the PCSP. PC will ensure each Basic Assurance has been reviewed to ensure the presence of each BA
4th Corporation utilizes data and informa-tion on key community issues. / Compile Basic Assurance data, Personal Outcome data and Incident Reporting data.
The public will be kept informed in hopes of gaining community partnerships that lead to employment opportunities. /
- Become involved in community issues
- Get 4th Corporation faces out there
- Community Pool member. Participated in cardiac ready community and have all staff trained on AED and CPR.
- Shred the R word. Look at getting into the school to present on disability awareness. Worked with the city on a fully accessible housing project that some folks live in.
4th Corporation will continue to be involved in the communities it serves. / Continue to provide free CPR and First Aid to the public.

- Continue to offer recycling.
- Continue to offer volunteers the opportunity to be involved in the community by being flexible with work hours. Continue to manage the thrift store so it is available to the public.

- Started pickup delivery of cans to increase awareness. Determine feasibility of recycling in New Rockford.
- Steam threshers, Adopt a highway, Eagles, school run programs, community ambulance, volunteer at thrift store.
- Revised PCSP agenda and meeting minutes to focus on community involvement when reviewing POI.
