Dear Parents/Guardians,
During the course of your child’s enrolment at Southern Cross Primary School there will be times when the opportunity will arise for your child to view DVDs of movies in the classroom. These may be part of the unit of work being studied or may be for special occasions, such as end of year celebrations.
These movies will either be G for “General” viewing or PG for “Parental Guidance”. If they are PG Rated movies, we require permission for your child to participate. Movies are always carefully selected by staff members to ensure they are appropriate for the students viewing them.
This consent form will cover the period of time your child attends Southern Cross Primary School. We would appreciate your consent by returning the attached form to your child’s teacher.

HEAD LICE INSPECTIONS ……… Parent Consent Form
The management of head lice infestation works best when all children are involved in our screening program. Whilst your child attends Southern Cross Primary School, there will be periodical head lice inspections of students, and inspections when individual head lice cases are reported. We ask that you give consent to these checks so that the presence of head lice can be minimized.
The school is aware that this can be a sensitive issue and is committed to maintaining student confidentiality and avoiding stigmatisation.
Before any inspections are conducted, staff will explain to all students what is being done and why, and it will be emphasised to students that the presence of head lice in their hair does not mean that their hair is less clean or well kept than anyone else’s. It will also be pointed out that head lice can be itchy and annoying and if you know you have got them, you can do something about it.
The inspection of students will be conducted discreetly by trained school staff members. Each student’s hair will be physically searched to see if any lice or eggs are present. Each child will then be sent home with written notification informing you that an inspection has taken place. The notice will indicate if eggs or lice are found and if treatment is required.
A Head Lice Information Sheet is attached to this notice, which outlines what to look for and how to treat the hair if necessary. Please note that the law requires that where a child has head lice, the child should not return to school until appropriate treatment has commenced.

PHOTO PERMISSION……….. Parent Consent Form
At Southern Cross Primary School, we sometimes take photographs of the students as they participate in school events such as art, sport, classroom activities, school leadership, excursions and incursions. We like to use these for school marketing purposes to promote our academic and extra curricular programs. These photographs may be used in school newsletters, school notices, notice boards,local newspapers and the internet.
Children are identified by their first name only. If you consent to having your child’s photo used for these purposes, please complete the form below and return to your child’s teacher.
This form will cover the period of time your child attends Southern Cross Primary School. If circumstances change and you would like to revoke your consent, please contact the office or your child’s teacher.
