Thanks to the program’s official partner MLC, we are pleased to be able to provide a MLC Tennis Hot Shots t-shirt to each registered participant in the MLC Tennis Hot Shots program.
To receive these t-shirts for your children, please complete your details below and return it together with a completed participant spreadsheet OR completed player brochures to your dedicated Community Tennis Officer in your State Association.
Please note: One t-shirt per registered child per financial year.

Terms & Conditions:
- The Deliverer requesting t-shirts must be registered with Tennis Australia as a MLC Tennis Hot Shots Deliverer at the time of request
- Participants must be enrolled in a weekly MLC Tennis Hot Shots program (coaching or competition) for at least one full school term to be eligible to receive a t-shirt
- Participants involved in the Tennis in Schools or Active After-school Communities programs are not eligible for a t-shirt
- Participants are entitled to one t-shirt per financial year (1 July 2010 – 30 June 2011)
- Quantities and sizes requested by the Deliverer are subject to availability
- T-shirts will only be provided if full participant details* are given by the Deliverer to the State Tennis Association together with this form. *Mandatory fields are first name, surname, gender, date of birth and child’s residential postcode.
- Quantities of t-shirts requested must match the total number of participants details provided by the Deliverer
- Please do not provide participant’s details who have previously received a t-shirt in this financial year
- Tennis Australia and the State Tennis Association are not responsible for any lost or misdirected t-shirt request forms
