Subject: Capital Safety Shock Absorbing Lanyard Product Recall

Capital Safety is aware of a reported inadvertent disconnection during use of the locking snaphook used in a series of “Y-shaped” Protecta lanyards with twin lanyard legs that both attach directly to the eye of the snaphook where it attaches to a harness d-ring. In a particular use configuration when the lanyard legs are pulled upwards simultaneously and the snaphook is compressed against the harness d-ring, the combination of forces acting on both the locking mechanism and gate opening of the snaphook may cause the snaphook to disconnect from the d-ring.

Although there have been no reported falls or injuries associated with the use of this configuration lanyard, Protecta part numbers AE57830 (2UZW9) and AE57831 (2UZX1) are subject to a voluntary recall by Capital Safety.

We have been working with W.W. Grainger Inc. who may have sold you the affected lanyard to recover any product you may have in your inventory. Based on W.W Grainger’s sales records, it is possible that some of the product you purchased between December 2007 and April 2009 is affected by the recall. To assist our recovery of the product, we request that you take the following actions:

1. Check any product in your possession by verifying the manufacture date found on the product label

2. Segregate and discontinue use of any product that displays a manufacture date between December 2007 to August 2008 (AE57830) December 2007 and April 2009 (AE57831)

3. Contact Capital Safety to arrange for returns of this product and replacement product at our expense by calling 1-800-328-6146.To facilitate the return of this recalled product, we ask that you review the enclosed Recall Notice.
