The Practice sought to engage a wide variety of patients on the Patient Reference Group. A message was put on the Jayex Board in Reception and also a Poster was put up. “Registration of Interest” forms (known in the Practice as Appendix A) were printed and included in the new patient’s welcome pack. The forms were put in all clinical rooms and given out by the nurse and doctors as well as being given out by receptionists. A poster was given to the carer at Yew Tree Lodge, a residential home for the elderly, for display on their communal notice board. A poster was also given to warden from Peter Lyell Court, another residential home in the area, and displayed. Patients who attended the flu clinic were also invited to join the Group. Forms were also given to Chimsons Chemist in Ruislip Manor.
Differences between the practice population and members of the PRG

The people who agreed to join the group were in the following age brackets:
Males- 17-24 X1, 25-34 x 1, 45-54 x 3, 55-64 x 2, 65-74 x 3 and 75-84 x 2.
Females 17-24 X1, 35-44 x 1, 45-54 x 1, 75-84 x 4
All people in all ages were invited but some very unresponsive.
Efforts to encourage the patients to join are listed above.

Stage two: Validate the survey and action plan through the Patient Participation Report

Survey
Please describe how the priorities were set
Practice designed it’s own short survey to gather views around the priorities and services provided by the Practice (please refer to Appendix B).
The survey was conducted for a week in November. All patients attending the Surgery were given the form to complete.
The Feedback obtained from the survey was presented to PRG by Sheila , as mentioned in below paragraph. All the priorities highlighted in the feedback from the short survey were included in GPAQ which was consented to by the Group.
Telephone System:Is highly ticked this year – (please refer to the minutes of meeting and action plan for response)
Opening times: Was also of highcategory and patients would like mixed opening hours especially weekends and laterevenings. (please refer to the minutes of meeting and action plan for response)
Reception issues: Targets dropped since last year – some patients indicated that their dissatisfaction on receptionist seeking reasons for appointments (please refer to the minutes of meeting and action plan for response)

Same as lat year – we did seek and inform PRG in one of the meetings (before conducting the survey) that practice will be using GPAQ again this year to analyse data. Agreed
Please describe how the survey was conducted?
GPAQ was distributed in the month of Oct-Nov to 130 (30 extra this year) patients visiting the practice. The distribution of the survey was done to capture patients visiting all the Clinicians.

Action plan
How did you did you agree the action plan with the PRG?
The results were discussed at the PRG meeting by showing a short PowerPoint Presentation. The highlighted areas of improvements were discussed to gather people’s suggestions to further develop the services.
Though the result have dropped since last year however the reasons indicated was acknowledged by the PRG and they commended the practice on delivering the excellent services. The Practice discussed the priorities that were highlighted via the survey and seeked PRG’s advice to highlight any concerns that were not incorporated. None were suggested.
