“The Division arranges for services to be given to any child up to 21 years of age who, with his parents or guardian lives in Illinois, provided the child has a disability condition, or condition that will lead to disability. No financial determination is necessary for diagnostic examinations in a division clinic. Consideration must be given to the family’s financial status in determining a child’s eligibility for further service. In planning further service, factors to be evaluated in basing conclusions on financial eligibility are the particular type if disability the child may have and the probable kind and length of medical, surgical or other care that might be required.”
Children are accepted for services who have: 1) Orthopedic handicaps, 2) neurological handicaps, 3) cardiac handicaps, 4) body deformities which are amenable to plastic surgery, 5) speech defects associated with congenital or acquired brain, or pharyngeal defects, 6) hearing loss and deafness, 7) rheumatoid arthritis, 8) trachea-esophageal fistula; esophageal stricture and 9) cystic fibrosis.
Referrals are primarily initiated by the doctor or hospital providing care to the child. However, a parent may call the Chicago office to initiate the application for services.

Supplemental Security Income (S.S.I) is an amendment to the Social Security Act, which provides for a federally funded and federally administered program of public assistance for the severely disabled and needs aged (age 65 and above).
A basic cash income is provided for persons whose disability has prevented them from gainful employment, and thus from earning regular social security. The following persons are eligible: 1) physically or mentally handicapped children living at home, under the age of 16 and if in school under age 22 (depending on there parent’s income, resources and size of family),
2) physically or mentally handicapped children in private residential facilities regardless of parent’s income and resources. Other persons are eligible, but not applicable to handicapped population.
S.S.I. is administered by the U.S. Social Security Administration, but it should not be confused with regular Social Security.
